Hi, Devon and Rhiannon and everyone who visited, but didn't post today. Not much to see here. PMPS was 102, which is good, and Max seems happy.

Max is getting fluids tonight. It is very hot and dry here after many days of unusual humidity. We are expecting Santa Ana winds -- winds blowing from the deserts East of us bringing more hot, dry air. It was 104 here today and should be tomorrow, too, and then, it will drop into the low 90s for three days and move back up to the high 90s for three days after that.

Max is sitting on Daddy's lap getting his ears, head and back rubbed right now. He's purring his little heart out.

Emma is fast asleep in her bed after falling asleep in the car and sleeping through dinner, too. She missed her nap today. We tried, but she wouldn't sleep at grandma's after the party. She had a great time at the party (which was for the 5th anniversary of a baby sign language class/group here in San Diego). We won two things in the raffle -- 4 tickets to the new San Diego Children's Museum and a free sitting for a Halloween costume photo session with a free 5 by 7 photo included. (How to get Max included in the photo session . . hmmmm?) We also enjoyed the kick off the party being her godfather playing guitar and entertaining all the little ones with the likes of Itsy Bitsy Spider and You Are My Sunshine as well as his own songs. (His big bi-annual festival is next weekend, and he was a little distracted today, I could tell, but I doubt anyone else could. Then, he goes to Austin for Austin City Limits October 12 & 13th. Hope any of you in Austin with kids will catch him in the Kiddie City Limits section of the festival.)

Wow! Is that after fluids? Looks like it might be another busy night. Someone might be on a mission. :-D

Hi, Wendy, Hey, Carl, yes I gave him the fluids just before I tested him for the 63. It takes about 6 hours or more to absorb.

It may take six hours total, but they start getting sucked up immediately. Depends on how dry he was:-) 100ml?
Did you feed the 51?

No, Carl, I didn't feed him, I can test again right now which is +1.75 or wait until +2. You think I should test, huh?

Almost, Carl, I gave him a teaspoon of HC! Something he never gets the first time he dips below fifty, but considering the fact that he will probably never have high carb food again ANTI-JINX!, I thought he would enjoy it, and I'd like to go to bed sometime tonight! :-D

I've never watched the day to day progress of going down to .1 and then starting the trial before this, Dale. But I guess you just "do it" once they earn that last deduction?

Of course, I am! :-D It's very exciting.

Eyeball .05, yeah, right! I actually thought about holding the plunger tight as I do right before I put it in the pen (in order to avoid air bubbles), putting it in the pen, and then letting it go. That, I think would be the tiniest dose one could get into these syringes. It would probably fill the needle, barely.

Hi Dale!

You have two options, as I see it. You can start the trial as of now, or - if you can be around to watch him closely for another couple of cycles, knowing you're almost there and you won't have to do it again *ever* if all goes as planned - you can give Max one last day on the 0.1u. This is only his 4th cycle on that dose, and the dip today *may* still have a little to do with the shed catching up (since the reduction from 0.25 to 0.1 is a pretty big one relatively speaking, 60% less insulin).

But it's entirely your call. I only bring up the option of shooting for one more day because we tend to try to give the insulin as long as it is at all possible, to make for the strongest possible remission. You really can start the trial NOW, too, if you want.

Whatever you decide, we're THRILLED for you!

Carl, we went from 0.1u to nothing, both times. In 2008, we had 4 days (8 cycles) on the 0.1u and when we saw inverted curves on three concsecutive AM-cycles, we decided it was time, with the wonderful advice and encouragement from Team J as they were then known. In 2012, we had 9 days on the 0.1u - but that was Karre's 2nd stint on Lantus, and a 2nd remission being more difficult to achieve, we took it very, very slowly to be as safe as possible.

Well, I am a little concerned that he didn't eat much today between 11 a.m. and 9 p.m. The wind slammed a door shut, and he was locked in the bedroom! :o Nonetheless, his BG was 102 at 9. I think this means all his organs are working well. Maybe it even means that he's ready for his trial?

I don't mind watching him another day, but I do hate pricking his ears. He really objects to it nowadays, and he cries out sometimes. I feel so bad when I hurt him. If he's going to go low because of the fluids I gave him tonight, I'd rather not give him insulin tomorrow morning. So, I think I'll take what's behind Door Number One!

Teresa, thank you. I have to laugh every time I see Poopy's name! I have a friend who named her cat Miss Poopie Poops-a-Lot, and I think of her every time, too. She's a very funny person, and I don't get to see her much any longer because I moved pretty far away.

Dale, congratulations on the OTJ Trial. I can't believe it! I am so happy for you! Glad you and Emma had a great time yesterday at the party. It was 105 here yesterday and low humidity.

Generally, we suggest you start the OTJ trial on your next green pre-shot number. Just so you have the guidelines for the trial, here they are:
  • Test at your normal AMPS and PMPS times. Feed multiple small meals throughout the day as much as possible (small meals are less likely to overwhelm a newly functioning pancreas).
  • If your cat is green at your normal test times, no need to test further until the next "PS" time, just feed small meals and go about your day. If he is blue, feed a small meal and test again after about 3 hours. If his number is lower 3-4 hours after a meal, then his pancreas is working!
  • After 2 weeks, if everything is looking good, we have a party!

Sometimes the trial doesn't work the first time and we have to give a little more support in the form of resuming insulin. It's not the end of the world if that happens, we just give him the support he needs. Our goal is a strong remission and it's better to take our time to get that than to rush into remission just to have it fail later on.
