8/3 Bailey UK advise please PM shot given late

I am so upset,my routine was disrupted today,and I have just realised that I forgot to give Bailey his PM shot,it is now 6 hours AFTER the time it should have been given. He is on 4.25 units twice daily,so working on he is half way through his PM cycle I have given him 2 units (this is just under half his normal dose) and intend to give him his normal dose of 4.25 units in six hours time which will be back on his normal AM shot time.he will be short by 2 units in the past 24 hours,I have done this because I dont want his depot to drain down too much as his numbers over the last two weeks have been a little higher than we have seen during the last 5 months .I thought it best to give him the late reduced dose instead of missing a dose.hope I have done the right thing,and that this is making sense,as it is the middle of the night in the UK,and I am so stressed and cross with myself. Hi Diane! I sure hope someone answers your concerns soon. Something doesnt seem right though with you giving 2.u and then 6 hours later giving 4.25u. That is 6.25u in a 12 hour span. It would make more sense to me to give the other half(2.25u) if anything. Idk..just wondering what anyone else thinks.

The quick answer to your question is, no...you should not give 1/2 a dose and then give a full dose 6 hours later. If you miss the scheduled shot, you either need to skip it entirely, or if your schedule will let you, shoot the full dose late and then adjust your shooting times. You can work your way back to your normal time 15 minutes per cycle or 30 minutes per day, so being off by 6 hours would take you 12 days to get back on your "regular" time. Being off by a little bit isn't as big of a deal, but 6 hours until you shoot again is too soon

If you can't adjust your schedule by 6 hours tomorrow and work your way back, I think what I'd do now is to just wait until +18 (from the time you shot today) and give the scheduled 4.5u dose....then you'd be back on your normal schedule
 
Diane - I agree with Chris. You do not want to shoot again 6 hours after your last shot. Then you have to worry about overlapping nadirs. It just gets messy. What you can do is wait 18 hours, which will be what was your old PMPS, and give the full 4.5U then. You can always shoot a bit earlier if Bailey is high and you want to gradually get back on schedule.

A number of people have used the 18 hour schedule if they have an event they have to and can't make their shot time.
 
Thanks Laura & Chris for your replying and advise,would it be possible to give him a reduced amount at his AM shot time,say 2 units then that with the 2 units I gave at plus 6 (when I realised I had missed the PM shot) would make up the amount of insulin he missed? I am very concerned as the last time he missed his shot (on vets advise after hypo a year ago) we had a really hard job getting his numbers back into control. Hope someone answers my concerns as he is due his AM shot in 45 mins. Many thanks
 
Thanks Wendy,am being thick here,can you explain what you mean 18 hours from? His normal shot time is 6AM &6PM I have him the half dose 6 hours late at midnight. Do you mean 18 hours from midnight? This would mean he would be getting very little insulin over a long period of time?
 
If you give a second shot 6 hours after the first one, you'll have to monitor closely for the first 6 hours (last part of last night's shot) as that would be two cycles in play at once. It would be easier to either wait for his normal AM+6, but then you have to make up the time.
 
OK, you shot at midnight. If you wait until 6PM, then that's 18 hours after the last shot. The other option is to shoot at noon and gradually work that back to 6 AM/PM but that will take a while to do at 1/2 hour a day.
 
Thanks Wendy,got it . Sorry to be so dense,but I have just woken up,and am really stressed by doing such a stupid thing.
If I can watch over him all day today would it be possible to give him 2 units at his normal AM time,and bring him back into normal dose at his PM shot time tonight? Would there be a danger to him?
 
Better too little insulin for a short time than too much for a second!

He'll get back on track soon...it's just safer for you to wait until 6pm (18 hours after this last shot at midnight) and get back on your regular schedule
