8/27 Yoda ambg 90 +7 107 OTJ trial ??s

Yoda had his dental yesterday. He was on such a small dose of insulin that I couldn't cut it in half, so he went without a shot yesterday. I didn't give a shot last night since I have read a lot about how anesthesia can lower bg's too. I figured I might as well see how he did today with no insulin, just out of curiosity, since his dose was down to 0.1u. He is hovering around 100 today, which is a bit higher than he was before the dental. I know the dental and ear cleaning process could have stirred up stuff for his system to take care of. He is currently on an antibiotic and will be starting ear drops tonight.

My questions: At what point on a 0.1u dose to you decide to do a OTJ trial? At what numbers during an OTJ trial do you determine that kitty needs to go back on a low dose of insulin?
 
Is this +48 for AMBG this morning for Oscar?
How is he acting now after his dental? Eating okay?
When was the last time he ate?

My Raja's first OTJ was aborted on the 13th day when Jill and Libby evaluated her numbers. Raja was drifting up into the mid blues..and wobbling a little. Usually, they recommend feeding and then checking the numbers 3 hours after to see if there is pancreatic activity , ie lower number. That's why I was wondering when Yoda ate before that +7.

Hopefully Jill or Libby will be by ....nice numbers Yoda!
 
I didn't really know how else to write that the 107 was taken 7 hours after the ambg this morning. The 90 was fasting. The 107 was probably 3 or 4 hours after eating last. The auto feeder opened at 9:30am and the 107 was at 1:30pm. BUT... I don't know who ate what at that time since I was at work. The ambg was taken at the same time as Oscar gets his shots (so 6:30am). He seems to be eating and acting normally since his dental. Still a little flitty about the ears, but he did have some major cleaning done. The ear drops the vet gave me for him do have steroids in it, so I really hope they don't effect his numbers too badly.
 
We are closing in on normal shot time for us. If he is still around the same number, should I give 0.1u? Should we continue with no insulin and go with a trial?
 
No advice from me, but I'll be watching to see what the knowledgeable people say.

Yoda had all those blues not long ago. I guess it's a question of whether the pancreas has had enough time to heal... and of course, what the +12 number is.
 
he is at 142 now.... I am going to go ahead and give him 0.1u. I am sure the antibiotics from his dental and the ear drops need time to work before we try another trial.

I am still curious about the answers to my first two questions though :-D
 
How long will he be on the ear drops?
You know he is doing great right now...so, if you see a rise in his numbers don't worry about it too much.
I think he will return to perfectly beautiful numbers once that course is done. Okay?
Glad he is doing okay...the ears are so sensitive. Well, they are taking the focus off his little mouth, eh?

About OTJ trials, here is the Tight Regulation Protocols sticky addressing Decreasing the Dose. The links have more detail. Generally for a new diabetic 7 days of mostly normal numbers, or dropping below 50 on 0.1u. There are other patterns that can sometimes be seen in the spreadsheet that might suggest that a cat could use some time on a smaller dose.

About resuming insulin, it's not really a specific number, we look for patterns. If you get a number you don't like, feed a little and test again in 3-4 hours to see if the number went down. You can always post for advice if you're not sure. Some cats bounce to blue a lot when they first start an OTJ trial, and you want to give them enough time and enough pancreas assistance to bring the numbers down on their own, if they can.
 
Sorry I had a rehearsal tonight and couldn't get a +1. He is getting Derma-Vet ointment drops for the next 10 days. The insert says it is anti-inflammatory, antibacterial, and anti-fungal.

At +4 tonight he is at 72. He is looking a little mope-y tonight and hasn't played since he came home yesterday. I am sure he is still not feeling very well.
 
The ear ointment contains a steroid as well as antibiotics. Some cats will react to the steroid component with higher numbers. Some kitties don't have a reaction. If Yoda's numbers start looking "off," at least you'll know why.
