? 8/2 Mo +6 344 - ED Update (Spoiler - he is fine :) )

Yesterday - First day home from the hospital

Mo seems to be very high today. He gets up when he thinks he's getting more food and literally FLOPS back onto the floor when he doesn't. I've been just putting food in his bowl whenever he hints at wanting more and he's been eating everything. He's drinking water and peeing and pooping well. I'm so relieved the poops are awful smelling anymore. Tim was telling me that he has cottoned on to the antibiotics being in the chicken pieces and is leaving them behind, so Tim was able to get the tablets in his mouth without a hassle. Mo is too high to fight back as it seems.

Mo has a check up with the vet tomorrow so I'm going to ask if we should decrease his gaba from 50mg. It might be a bit too much for him now. 25mg might be enough. He's still not grooming at the moment. Well, he's grooming his arms but that's about it. He has a lot of food all over his face that we need to help him clean. I do think it's just because he's too high to do anything so a reduction in his gaba might be a good thing. Otherwise, he's doing really well. He was giving Tim purrs this morning but he seems to sleepy for purrs at the moment. He might have been giving me some quiet ones but he certainly doesn't seem uncomfortable today.

It does sound as if the gaba could be reduced.
I would definitely get some BG tests in and the ketone urine test. Otherwise it’s just head in the sand. You need to be knowing what is going on. I am concerned he is only getting the 3 units of insulin still....
Im very glad he is eating well and happy. :):bighug:

We have ketones. (The second one is the ketone test). I can't tell it's it's the the + or the ++. But I will definitely be complaining to the specialist tomorrow AND the vet about lack of insulin. This is not ok. He's still eating and drinking really well. Should this be something I need to act on now?

Hi Katy - I have zero ketone experience but wanted to as least share the ketone document in case you haven't seen it. If it were me, I'd at least be calling my vet. From what I understand, ketones can progress quickly. Sorry I can't be of more help but at least this will bump up your post.


 
Also, what should I increase in insulin to for his PMPS shot? Currently on 3 units.

Do you have any subq fluids there that you can give him if he has no known heart issues? Even 50 mls would help. If not, he needs as much fluid as you can get in him via foods or chicken broth that has no onions, soy, garlic, etc.

How long before your shoot? I’m talking to a few others about getting his dose back up quickly.

I don't have anything like that :( Should I call the emergency vet? We shoot in just under 5 hours so lots of time.

We’ve had members handle trace ketones at home but ketones can develop rapidly. Fluids can help but he really needs more insulin. It wouldn’t hurt to call the ER vet if you trust them.
 
I just called the after hours vet (it's a public holiday today in New Zealand). They said they will get back to me within the next half an hour.
Please keep us posted and get us much fluid in him as you can. How is he acting? Is he eating and alert..moving around?
 
Here’s what I’m thinking....the info I have on ketones and what we’ve seen is that ketones above trace shouldn’t be managed at home and if you can get him to the vet before his electrolytes tip, all the better. The other thing is they can give him some R insulin to get his BG down a bit. The issue we have with just putting him back on +9u of Lantus straight away is we have no current data. He could be in the 500s or 300s. You did give him 6u and he did ok so that is a possibility. We could give him a couple cycles at that and then increase more quickly.

I’m waiting to hear from Wendy because she deals with high dose cats more than I do. I would feel better if she had some input but I would really press the vet about the possibility of giving him some fluids and maybe some Normalin R to get the numbers down. We can then fast track the L dose back up.

Can you get a BG now please?
 
I'm taking him into the vet now. I'm alone at home and I really don't know what to do so the vet it is. Tim has been out and has had beers and i'm so mad i'm having to do this all on my own.

She checked all his vitals, everything is going well. He doesn't appear to be in much pain and he is still very chill from the gabapentin from this morning. She took a urine sample and she could hardly see any ketones! She said she had to squint to see if there was any there. So at most it's trace ketones. She said Mo is well hydrated, which is also good. The vet didn't want to admit him to the hospital as he would get too stressed and as he is well hydrated, she didn't see it necessary. Thank goodness :D

As soon as we got home, I gave half a can of FF and he went straight to eat it. He's still a bit stressed so didn't eat it all at once.

I'm so bloody relieved, I can't even tell you. The vet there said that we know our cat better than anyone else, and we should increase to 4 units if we feel it would be best. FINALLY someone not afraid to go against the recommendations of Prue, the specialist!

I need to relax...maybe have a beer....

That’s great news but boy it sure didn’t look like trace on your strips. I’m so relieved and glad he’s eating.

Wendy might post but I had suggested you go back to 6u and she would like to see you go straight to 9u since you have time off. You up for that and starting to test again?
 
Glad Mo is back home again! It did look quite a bit more than trace. One thing to keep in mind, is that you need to see the test colour at exactly 15 seconds. The strip can change colour beyond that after 15 seconds, which would be an incorrect reading.
