Re: 7/9 Chuck 544 AMPS; 345 +2

Could he have gotten into any high carb food? That seems like it came out of nowhere. Your cycle last night looks pretty normal.

Re: 7/9 Chuck 544 AMPS; 345 +2

No chance on the high carb stuff -- they all eat Chuck's food, and the treats are behind closed doors. Unless he ate a spider or some other bug that was high carb <smile!>... I did shoot a skinny 1.2U last night, between 1.0 and 1.2. Probably should have stayed with the 1.2, but he was below 400 at preshot.

So, should I just hang on to the 1.2U and ride out any highs and lows, or should I continue to adjust based on his preshot number? He's been back on insulin for 4 1/2 months and I'm still not seeing stable numbers.

Another question for all of you out there -- if Chuck were your cat, knowing his ups and downs based on his spreadsheet, should we be looking at PTS? He is 18, will be 19 next March; seems to be in good health other than diabetes (no liver or kidney problems that we're aware of based on testing), although he does seem to be a bit dehydrated based on the pull out the skin and watch it rebound test. He eats well, and is still social; usually purrs throughout testing if I catch him at a good time. I try to spend time with him without pokes and needles, but it seems like more and more of our time together is pokes and needles...

I'm a fan of the sliding scale.

As far as PTS if he were mine, I wouldn't do it. If he's happy, has no other health issues and his 5 P's are mostly in place dealing with the FD isn't that big of a deal. He spends most of his days in blue and green numbers. If you want to work on the higher PS's we can do that.

That said, only you can make that decision for Chuck. You would know best what his quality of like is.

Am I on the right track, sliding scale wise?

1.2U >400 preshot
1.0U >300 preshot
.6U >200 preshot

Or should it be a tighter scale?
1.6U>500<550
1.4U>450<500
1.2u>400<450
1.0U>350<400
.8U>300<350
.6U>250<300

I would go with the first scale for now. I tried to put together a scale but with the bouncing I can't find a good starting point. Maybe the first scale will stop the bouncing and give us a better understanding of how the doses are working.

Ok so I went over your scale again this morning while I'm a little fresher minded.

Your tighter scale is actually pretty good but I would hesitate shooting a 1.6u mostly because he can have a better than expected response to a 1.4u dose and most of those higher numbers are bounces and you can't always predict when they will clear.

So here is the scale I came up with, it is a little more aggressive than you have been shooting but you are able to get mid cycle tests and can stay on top of any lower numbers if they come up. Mind you that a more aggressive scale could cause more bouncing, but maybe he will get used to it faster if he has even lower nadirs. Don't know.

Chucks Scale ONLY>

200 to 249 - .8u
250 to 299 - 1 unit
300 to 349 - 1.2u
over 350 - 1.4u

Each step of the scale can be adjusted if we see that it isn't working like it should.

Sounds to me like Chuck still has a pretty good quality of life. Yes, you are doing pokes and prods and shots, but that is strengthening your bond.

When the 5 P's go down hill, and the appetite declines no matter what you try, then it may be time to think about ending Chuck's life. You are nowhere near that point, IMHO.
