Good cycle last night, I guess it's safe to keep her at 0.1 for another little while. Seems good bc I was nervous about starting an otj trial so soon. This cat has exceeded all expectations.

Gonna nap another hour then report +1.

looking good. last night's cycle was good too.

i think it was a good choice to go ahead and give her insulin last night and this morning. we want her to go OTJ and stay there when she does! :mrgreen:

Her +2 is 50.4. Right on the edge... I gave a little HC mixed with LC and will see where that goes.

Well she hit 48.6. I'm a bit sad because was hoping to give her a couple more days on insulin - but this seems like a signal to stop. Do people agree?

I also won't be around to monitor tomorrow.

Do I start the trial tomorrow?

I'm not able to give dosing advice but if you can't monitor tomorrow I would think your trial begins tonight because there will still be a depot. One of the experts will surely advise you before PMPS which will likely be PMBG. Congrats. Don't be sad. Be happy. When a cat is done is the best time to stop. ECID.

Haha thanks tiffmaxee... I am definitely more happy than sad! I was just hoping for a couple more days of safe surfing. But this cat has an agenda, it seems.

My $0.02
The general guidelines for reductions (newly diagnosed kitty) are
a)if BG drops below 50 or
b)if you have to work really hard to keep kitty above 50.
This cycle has both those elements.
Looks to me like the new dose is 0u :mrgreen:

Did you last feed at +3?

I last fed at +2 but she ate a little more between then and now (+4.5). She's a snacker, not a hooverer. She's now at 57.6.

Wow, she sure told you what she wanted right away, eh? Isn't it funny how quickly they can run the other way from the insulin just when you finally feel like you may have a handle on the whole process? I'm really hoping that the OTJ trial goes perfectly for you!

How often should I check bg during the trial?

We suggest posting daily with her AMBG and PMBG numbers. That way if anything seems to be anything going off, we can provide suggestions. Congratulations and good luck with the trial!

ETA: Check her AM and PM BG numbers and if they are green you are good to go. If blue, check again 3 hours after she eats.

Here are the trial instructions which should answer all your questions but if it doesn't, please let us know.

Normally we would start the trial on her next green pre shot so keep that in mind. If she goes way up into blue, post before you decide what to do but if she's 100 or lower, start the trial.

If she is green at your normal test times, no need to test further until the next "PS" time, just feed small meals and go about your day. If she is blue, feed a small meal and test again after about 3 hours. If her number is lower 3-4 hours after a meal, then her pancreas is working!

Post every day so we can monitor your progress and see if any tweaks are needed. It is not unusual for cats to get lots of blue during their trial, especially at first. Don't panic and don't shoot unless we tell you to.

After 14 days of no insulin, we have a party!

Sometimes the trial doesn't work the first time and we have to give a little more support in the form of resuming insulin. It's not the end of the world if that happens, we just give her the support she needs. Our goal is a strong remission and it's better to take our time to get that than to rush into remission just to have it fail later on.

If she gets through the trial successfully, you enter a new phase. Your cat is still diabetic, but has now become diet-controlled. Continue feeding low-carb food and small meals for the rest of her life. Avoid medications with sugar in them and steroid medications unless they are medically essential. Continue testing blood sugar weekly for the next few months and then monthly forever. It's a good idea to weigh her monthly. Her weight should remain stable. If she seems "off" or sick, or is showing symptoms of diabetes (excessive drinking, eating, urinating, weight loss) test her blood sugar right away. Keep the teeth clean; dental issues can bring a cat out of remission.

If you see rising blood sugar numbers, post here before you restart insulin and ask for advice on how to proceed.
