Hi Community,

After a disappointing visit to the vet on Monday where he told me to continue only a shoot per day, yesterday I asked for advice in this community.

Originally Luna was only receiving one injection a day of 0.76U. Base on the consultation I did in this community, the suggestion was to start with 0.5U twice a day.

Last night 7:30 pm (PMPS), it was her first second shoot of the day. Before the shoot, her BG was 198. I injected 0.5U and fed her.

As usual at 4:30 AM today I got up to feed all the cats, including Luna. At this time I did not do the BG test because it was so early, but she ate her food with a great appetite and I did not observe any signs of hypo on her, she acted as usual. Since we started this journey with diabetes and due to her neuropathy, I serve her food in her bed. :)

This morning, 7:20 am (AMPS), before her morning shoot her BG was 139 so I decided not to give the injection. I offered him food as usual, but this time she didn't want to eat. I guess she was still satisfied with her 4:30 am meal.

At this point, I don't know what to do. Should I re-measure her BG and if it is above 200 apply insulin or in any case just monitor her BG and only apply insulin when the next cycle comes at 7:30 pm today?

For general information, she was diagnosed 3 weeks ago, the first week she was eating Glucose Support from Hill´s as per vet recommendation. Starting on week 2 I began to feed her wet food, but still giving her the dry food as her first meal of the day. But with all the research I did in this community I move to only wet food since last Saturday. She is eating a combination of Nature´s Logic Chicken, Instinct Rabbit, and Friskies canned food.

You can see how her number has behaved since she started with insulin.

I will appreciate your suggestion on what should I do.

I’m glad you found us as Lantus needs to be dosed every 12 hours. The dose is also based upon the lowest bg in the cycle and you don’t have any mid cycle tests at night. You need at least a before bed test. The food change away from dry sometimes quickly helps bring cats into remission. Can you monitor as long as needed today? Do you have some high carb food like gravy lovers or karro or honey should Luna drop lower? Since she didn’t eat that is good. Get another test without food and post results. Then we can decide what to do this morning.

Yes I have honey and also the dry food she uses to eat ( Prey from Taste of the Wild) plus another medical paste prescribe by the vet ( I don't remember the name, I will check). I will test her as soon as I finish a work call I'm in and inform back in here.
 
If you haven’t injected and haven’t fed I suggest you test and decide as you are going to be off schedule.

I have just tested her and is 143. I offer food again and she didn't want it!! Should I inject her now?
 
Is it unusual for her to not eat? Lantus won’t kick in for at least an hour and usually two but if she won’t eat I’d be concerned.

Before she was diagnosed she use to eat at 4:30 am, 7-7:30 am, 12 md, 5pm and 7-7:30 pm. The last few days she has been eating less, if she eats a good meal at 4:30 am, she is rejecting to eat at 7:30 am but she start asking for food before 12md.
What do you mean "if she won't eat I'd be concerned."? Remember she ate with great appetite this morning at 4:30am.
 
Then as long as you can monitor and alter your schedule you can shoot. You will need to shoot an hour later tonight. I’d leave some food out now for her.

I'm working from home, so I can monitor her as frequent as I want.. I just left her food next to her bed and will check-in 30 min if she ate.

At +5 (noon) she was on 124. She refused to eat during the morning, but at noon she ate 1/3 of her canned food with great appetite.!!
 
It’s good you shot but was it early? Has it been 12 hours? Please get a +1 and +2 if yiu shoot early as if less than 12 hours it can act like as increase. Eating ok?

Yeap I forgot to delay the shot 1 hour.
About eating, at 5pm she refuse to eat, she tried just some little bites. But at 7pm she ate. I put a little of chicken broth to estimulate her to eat. I hope it is ok to put the broth on her food, i did not put any seasoning when cooking the chicken, just water.
 
You need to get tests at night or it will be impossible for anyone here to give dosing advice. At least a before bed test is necessary. Read the dosing methods also to see which one you will follow. Having a vet giving once a day dosing advice means that vet doesn’t understand how Lantus works so you can’t rely on your vet.

I know I need to start testing her at night also. Do you think testing her on +9 after her PM shot it ok or it would be better on +2. The issue is I'm giving her PM shot at 7 pm approx. and I go to bed at 8:30 pm because I wake up at 4:30 am to feed the cats and then go to the gym.? Or I should wake up at midnight to measure her BG?

Luna is getting so stressed any time I need to poke her ear that I was hoping that testing her only before her shots will be enough but I will do what it is better for her. Thanks in advance....
 
If you can possibly get a +2 at night it will alert you as to whether it might be an active cycle. If she has dropped or is the same as the PMPS you would expect her to possibly go low during the cycle. If active another test at midnight might help figure her out and other times you can just test at +9. It sounds like she’s not food oriented but a treat after each poke if you can find something special might help. Either freeze dried chicken, shrimp, or any protein she likes or a bite of plain chicken might work. I envy you being able to go to the gym. Covid is preventing me from safely going and I’ve put in the Covid extra 4-5 pounds. :(

I start giving her plain chicken ( cooked) after testing a per reading in one of the post on the Feline Diabetes Fb page, she loves it, If it were for her she will only eat that chicken.
