7/09 Kozmo, AMPS 214, +1 347, 2wks Post DKA

He could well drop down into greens again today so I’m glad you are getting a +1 and+2 to start.
BG at +1 is 347 and I suspect that’s indicative of his breakfast. Is that a normal breakfast outcome, do you think? If it’s higher than expected I may need to check the carb content of the food (it’s his pre-diagnosis wet food that we’ve been feeding him in the dodgy post DKA period). It’s 7.097% on the carb calc app but I’ve heard that it’s not always accurate.

A question re timing: This morning for example, his AMPS was at 8am and by the time he ate and we shot it was 8:20am. I tested his +1 at 9:20am. Is that correct? With Caninsulin we had to wait 20-30 minutes after feeding to shoot, can we shoot straight after AMPS and PMPS and then feed?
 
With Lantus it is always test/ feed / shoot in that order. I used to give Sheba her insulin while she ate because I knew she always would eat ... except if she had a pancreatic flare and then I’d wait to see she’d eat. If AMPS is at 8 am you can test at 7.50 then feed and you will be shooting at 8 am.

Most cats have a food bump @+1 but the 347 is quite a bump though. Carbs of 7% is fine. At the moment what he eats is not as important as that he does eat.

Thanks Bron. We’ll tighten up the window between test, feed and shoot. He refused the LC wet food we put down (though he did eat a pouch of it throughout the night) so we gave him his favourite pre dx wet food. I’m going to check the Whiskas website to see if I can find the real carb content.

It’s officially two weeks today that Kozmo was DKA and exactly two weeks prior to that was DKA and diagnosis. Between Kozmo and the support of his GA’s (and our efforts too) he’s in much better shape now.
 
BG 382 at +2 but only moments before that he fainted. He was lying in my lap at the time and I managed to snap him out of it within a few seconds. It was quite frightening at first because it was exactly at the +2 mark and I thought he was hypo! We suspect this is gaba related because we missed two doses and this hit him hard, even though he’s on the lowest dose. Lessons learned.

Is he eating ok? And still no ketones?
He’s eating well (I plonked him in front of his bowl just now and he ate over half a pouch). No ketones.
He fainted? Are you sure he didn't fall asleep? That would be more likely with gabapentin.
Hi Sienne, he definitely fainted. My husband had just scratched his chin and moved away, then Kozzie’s head fell to my lap and his body went limp. I immediately gave him a shake, then picked up his head it dropped straight back to my lap. He was dead weight when I picked him up. He came to after a bit of yelling his name and gentle prodding. Poor thing was completely dazed. Definitely not asleep. And definitely not his normal gaba nap. However! He’s completely normal now and eating well.

Sienne, I need to reply to a comment you made in my thread yesterday. Because of our time difference, it was in the previous days thread and I’m not sure how to reply to it here. It was in relation to vets. I’m Australian and our family used to use the cat specialist in Mt Gravatt Queensland many years ago. My husband and I have since moved to the U.K. - Scotland first and now Plymouth - with our fur babies and we’re finding it impossible to establish a rapport with our vet (any vet to be honest). In Australia I always had a great relationship with our vets, but over here it’s difficult to find someone that even listens to us, let alone proactive in establishing a rapport. It’s text book veterinary practice and that’s it. From our experience with U.K. vets so far, that is.
 
BG 304 at +5 maybe indicative of his big feed after +3? Need to start moving him off his pre dc wet food.
He might be still bouncing up and down a bit from the blue BG yesterday morning. They don’t always just stay up high. If he’s eating the pre FD wet food well and you think it’s about 7% I’d be just really happy he’s eating at this point. Cats often do better with a few carbs (around 5%) than no carbs.

Very good there’s still no ketones!
 
Glad Kozmo was feeling better this morning.

Have to say, I find the fainting extremely worrying. Between that and his crash the other day, it really seems like there's something else going on with him.

I can't remember, but has he had his potassium levels checked recently?

I wish you had a decent local vet to consult :(, I'm at sea with these kinds of problems. I don't even know what kinds of conditions to suggest to ask about but I do find the fainting and the weakness the other day extremely worrying... What about local ER vets? Not my usual recommendation, honestly, they're so expensive and not always great, but just for a quick second opinion without needing to get into a full relationship with a new vet.
 
There was a woman who is in the UK who helped a lot on the main forum. Liz I think. Maybe post on the main forum asking for vet suggestions close to where you live. This sounds like a seizure possibly or a reaction to medication @???That must have been so scary. :bighug:
 
Apart from Kozmo’s fainting stunt this morning, he’s had a good day. He slept a lot which we attributed to Gabapentin, and even then, every time I woke him up and put him in front of food he ate a decent amount. He ate at least 3 x 85g pouches and perhaps another overnight.

The vet was happy with his potassium levels. We test twice daily for ketones and they’re negative.

This evening, although wobbly from gaba, he’s in really good spirits. He just chased me UP the stairs - haven’t seen that for so long. More grooming, purring and asking for cuddles.

Tiff, you’re right - Eliz (& Bertie) in the main forum is a wonderful contact for vet suggestions.

Sienne and Wendy, thank you for the heads up on the RVC feline diabetes remission clinic. I downloaded their app when Kozzie was first diagnosed and it’s a brilliant idea for me to reach out to them.

Hi Bron :) From memory (not working quite so well recently :facepalm:) Eliz is from Surrey and may not have any suggestions for Plymouth but one never knows who she may have come across in her FDMB travels ;). I’ll ask but am not the type to hound anyone (I hope!). Sienne came up with the brilliant idea to contact the RVC and I’ll definitely do that.

Neil and I are hoping that if we can prove to our vet how well Lantus is working for Kozmo (we’ll give it much more time to improve), maybe we’ll have earned some respect instead of distain.
