Hi

Can someone look at Jake's chart and advise whether or not I should increase his dose by .5 or not? I have problems deciding when I should adjust.

Hi Cory and welcome to you and sweet Jake here. Is there any chance you can get a before bed test at night? We don't expect you to always get up for a night time nadir check, but a test before going to bed will tell us a lot. Without any tests at night you are missing half of your data. It's possible for Jake to be going low at night then bouncing during the day which means he possibly needs less insulin. Or he could just need more insulin. Without data for every cycle, I have a hard time suggesting what to do with the dose. If you could stay with 2U and get a couple of tests in at night as well as the preshot tests, that will help.

I also noticed your comment that you ran out of strips one day. For that reason and for cost, most of us just buy a human glucometer and strips. The strips are much cheaper and you can usually get them 7 days a week and quite late if you need them. The protocol we use here is calibrated for people using a human blood glucometer.

I have to agree with Wendy. Cory is already seeing some blue numbers at PMPS. If you can squeeze in a test at PM +2 at least you'll know if Jake's numbers are dropping and if you need to get some additional tests during the PM cycle.

I noticed that you ran out of strips. You might want to consider using a meter other than an AlphaTrack. The strips are far more expensive compared to a human meter and if you're low on strips and your vet's office is closed, you're out of luck. Most of us use a human meter -- it really helps with cost.
 
I have to wait until I start my new job in two weeks to be able to get a human meter. Right now I'm unemployed w no income, and can get his strips on my care credit card.

I can do a test at +2 since I don't go to bed until 10pm.

Welcome to LantusLand, the greatest place you never wanted to be. :lol: :lol: Ah, well, we are all crazy about cats and only want the best for them. Sienne and Wendy are quite experienced and give excellent advice. I will also add that we generally change the dose by 0.25u at most, sometimes even less. Kitties can be very sensitive to even tiny changes in the dose so the protocol calls for small changes.

We started out using the AlphaTrak meter, too and switched after about 6 months. I was very hesitant to change but was very happy when we did. A human meter is easier to use and much, much cheaper. I think the readings are just as good as with the AT. Of course, it's your decision and we will be quite happy to help you whichever meter you use.

The +2 test is a good one for Lantus. It gives us a heads up on what the rest of the cycle might look like. If the +2 BG number is similar to the preshot, that means it's going to be a normal Lantus cycle. If it's lower, then it's likely to be an active cycle where you might need to get a later test.
 
Try a couple more cycles at 2 U and get another night test tomorrow so we can fill in the blanks.
