On the 16th, those 500s scream rebound to me, but data from earlier in the cycle is missing, so we don't know how low she went to know for sure like a Valley Girl (for those of us who grew up in the '80s).
There's the conundrum with Levemir that if dose is just a little too high, it hangs on to high numbers, making it look as if more insulin is needed over a week or more, like you are seeing.
But a 500 on Lev usually has no other cause than either a carb load (got in to the bread or dry food) or rebound.
So I agree with Sheila.
