Thank you everyone for your input,relaying your experiences and most of all the support and hugs. This place is the best for many reasons.
Well, the BIL is gone. Long story short - DH threw him out. There was instantaneous relief felt. Jones finally came to sleep with me last night, first time since BIL arrived. All cats were lounging on the floor this morning after breakfast. Good signs. DH is upset but agrees it is for the best. Alcohol can be such a poison.
Jones is totally back to normal now. Walking fine. Eating ok. Alert, cuddles, purrs. His BG are still higher than I would like in the last few days (disregarding the horrible numbers from the contraband and vet visit). Will give that another day and see with the BIL gone how the numbers go. May be looking for some dosing advice in the next week or so. It is a new batch of pred too, sometimes wonder if they are ever really consistent.
Just talked with our regular (maternity replacement) vet. She really thinks it was a clot as well. She thinks that maybe a BP check would be good and a ECG. I am going to take a few days and try to decompress and work out a schedule before booking those. I am concerned as if it is heart issues and he can't have pred - he has already shown that the bude does not work for his IBD - where does that leave me?
He didn't check his leg temperature. Outside of taking his normal temp which was normal.
There was no murmers/heart rate issues noted upon exam but that was noted when he was first sick before all the dx of pancreatitis and IBD. By then he was so thin, dehydrated etc. Our regular vet did say it disappeared once we had things going smoothly as she was checking for it. She never raised a concern of heart issues and the pred, I will need to ask the IM on that. The dose increase in pred has lead to an increase in weight and slight increase in appetite, 4.652kgs now! Was 4.45 before the change in pred dose and 4.1kgs before the weight loss started to get reversed.
I have tried to drop the metronidazole a few times now. Within 3 days he is back to loose stool - I am not sure with all the contraband instances, the stress of BIL or the removal of the metro - what is causing it.

. All I know, is when he is on it he may be a little constipated or he can be off it and have diarrhea.
So what evil do I chose? Again, taking a few more days to look at things and see where things are at. Spend the weekend camped out at the litter box campsite hoping to catch some action.
