6/23 ELIAN: AMPS a scary 595; 417 +5.25; 427 +8 Suggestions?

Hi everybody - yes I've been busy working and haven't been posting. I also haven't been getting any middle if the night tests. But Elian has been really high lately and I'm not sure why. I upped him to 2.5u yesterday and he woke up with his highest number ever. I made sure he was coming down thru the day but his lowest was still a sky high 417. Any ideas what may be causing this? He's acting fine. Even thought the insulin still has an odor could it be no good any more? It's several months old but my vet said I could use every last drop. The vial is almost empty and I have a pen and another vial. I'm guessing I should use of them tonight and see if it makes a difference.

Okay I'm rambling. Sorry! Hope everybody's enjoying their summer. I can't believe it's almost 4 th of July already. Wow it's flying by!!!!

Sometimes the last dregs of insulin seem to lose their punch. I'd try new insulin as a first step. Obviously, without the PM tests, there's the possibility of a bounce -- and Elian's good at those! The other things to rule out are any dental issues or just the need for a dental or if there's a medical issue going on. Sometimes, BG is the first clue that there's an infection brewing.

There's always the possibility of new dose wonkiness but I think Elian's been at this dose for enough cycles that it should have passed. OTOH, ECID and NDW could be what's happening. (And do you think I could have gotten any more abbreviations in that sentence?)

Thanks! He just had a dental a few months ago. He wouldn't need another so soon, would he? I will try newer insulin tonight and will make a point of getting up to test him. I'm so happy I've been around long enough that I don't have to ask what the abbreviations mean!!!! Thanks once again for your help. If he remains high tomorrow I'm thinking a visit to Dr.Melissa might be in order. :-(

Hi Susie--

I'm sorry to see Elian high.

If you are still using the vial from January, that would mean its been 5 months now, correct? If it is 5 months old, I highly suggest throwing it out and using the new pen. There's a real good chance that the insulin in your vial is losing potency.

The high numbers since the new dose can be "NDW"--new dose wonkiness. It can take a few cycles before you may notice a difference in BG's. I still would toss the vial if it is 5 months old.

I do hope he comes down. Be sure to check for ketones, daily if you can.

Have you been versed as to how to use the pen with a syringe???

One more question - when my daughters boyfriend heard the 595 he made me feel like maybe I should've run him to the dr. Is there a critically high number that I should be aware if? Like I said, he's acting the same as always, is eating, not acting any way out of the norm for him. So just wondering for future reference.

As long as you are getting ketones tests when he is high and he is still eating well, acting normal, and not staying at the high numbers, it should be OK as a temporary thing. Back in the day, Neko hit Hi (over 600) a few mornings in a row. :sad:

Thanks everybody. As for using the pen - don't I do it the same as a vial? Using my own syringes?

The pen is based on negative pressure, so you don't inject air into it first. You do still use the same syringes. You might want to check out the video on the Lantus Info page. The video shows both vial and pen.

Thx. I will check it out now. I've never put air into a vial - have I been doing THAT wrong? I followed the video on the board after I was told everything the initial vet taught me was wrong!!!!

Wow. Just watched the video again and I don't know how I missed the injection of air into the vial. Haven't Ben doing it all along. :-(
