meant to post yesterday.. things a blur sometimes.

so peanut has been sitting in his box on and off since last night. 🫤 he didnt eat too good (or get fluids) so i halved dose.. and he shot up to 200 for some hours 😭 but then things came back down.. used to be able to halve or skip and numbers stable, tho not intentional or desired.. i dont understand anything.. patterns change it seems.

anyway, wasnt long and numbers got better. ❤️‍🩹 and going back into the box off and on.. thing is, hes acting and behaving whatever the new normal is we got rn.. things otherwise relatively normal. he is eating! (so got his dose!), he’ll get out to drink water, he’ll chill out, he purred for me in the morning when he was getting pets when we were by the fountain (i help him drink a lot of the time…. he prefers now for me to pour it straight frm the bottle 👹.. and i was gonna say he had actually stopped purring that i noticed for a week or two since things dropped off this most recent time.. but i heard him start up again a day or two ago!), but hes off and on back in there…… im assuming UTI?.. worse? idk. :( i dont think he feels good at this particular moment..

i’m starting to think its time or something.. or maybe he has something not too serious… idk. 💔

made an appt at a place nearby w Saturday hrs.. cause of course this always happens at night or off hours….. we used to go there but it got stupid expensive. i just called too about tests and they are more than $100 over than other places…… the shame. im gonna see who else we can get.. but do have that if we have to. everything hasnt been cheap.. 🙃🫠 .. but ok to do it if he needs.. they just take you for a ride!!!!!

….sigh. ill see how he does til the appt. theres also an emergency i can take him to as well thats priced comparably… either way... ☹️

Update:

so peanut has stopped sitting in there the last couple hrs……. 🤔 i did change it tho; because if he was gonna sit in it i wanted it as clean as it could be. had some spare boxes. new bag of litter. since we changed to paper, i did initially have to pour some of the old clay litter in cuz he was a bit confused w it; he took to it tho right away when i did it (litter instructions said to do this too) ive been steadily decreasing it (clay) and since i seen him hanging out in there i just decided to change it.. that clay turns to straight cement!!!! 🫨😖

But maybe has something to do w it.. staying out? he did go potty (!) then got right out.. so eh?

shrug? ugh. 🫠

he also missed his +2-3 snacks which is a new thing to ever happen for a long time cause he hasnt been wanting to eat at intervals (even w Mirtaz.. i was thinking something wrong w the new tube we coughed up and bought, but he was feeling that last night too when i thought of it.. overnight.. today was last day on old tube. and ohhh!!! the new one, blasted thing squirted out half the damn tube by itself when i was dosing today.. was over $50!!!!… i gotta tell them; thing is watch em say outta luck.. 😑..)

he did eat his +4.. 🫠 you know i been killing myself w his snacks since we started frontloading etc.. maybe didnt have to?…. Idk. 🙃🙃🙃🙃 i always heard its good to do that and also feed greens… ugh. is that ok to do then?.. not always give a snack? just trying to get through.. i guess we gauge like we been doing.

And that he used his box for potty, too. Good news.
Ugh, sorry about the damned tube shooting out the meds.
Do you get it from the vet or chewy? Check prices, they may be less expensive.

guys.. his numbers are really bad. :( everythings just gone to the underworld.. creatinine and BUN.. it was done in house so theres not value.. theyre trying to get us one….. hes so anemic now…. Hes at 16….

doc thinks maybe Telmisartan did it cause he was stable before we started.. tho high BP is bad it can also help cause the kidneys get more blood….

im being told to stop the Telmisartan, see how he does.. but its soon. if it were them, they’d be making a call to someone to ease him.. but i could try stopping the meds and see if he maybe improves. otherwise i could just find him gone one of these days.. urinaylisis was ok.

hes sitting in there cause hes weak. doesnt feel good.

im in shock.. but not. i dont want him suffering. 😢
 
his phosphorus is higher than 16.. :(
He probably doesn’t feel good with that number. I see you are already giving him aluminum hydroxide. Did they put him on something for the anemia like Aranasp or Epogen? That low of a hematocrit can make them just lie there.

Keeping you in my thoughts. I’ve been down this road and I feel for you. The two things which I think are most important to stay on top of with CKD are phosphorus levels and anemia. I’m at least very glad he peed. 🫂🫂🫂🫂
 
Update 2 ..

so.. the vet visit went really nice consdering and what was found out is.. horrible. the vet themself was very nice and so was the tech. they were really compassionate and supportive.. I found out some unpleasant things, like getting bloodwork shouldn't affect anemia too much, the Telmisartan may be why we're here (because he had been pretty stable the months prior; I think we started the Telmisartan sometime in May.. the Internist also mentioned in their report too that hypertension sometimes can help values because more blood is being pumped through.. tho hypertension is bad...) They had a case where a dog of theirs went on it and the kidneys tanked, then went off and got better.. they thought things are looking weird too because his kidney shouldn't be so bad considering we do sub Qs everyday.. and that if it were their cat, they'd be calling the number they gave me to take care of things before he got worse. they suggested only essential treatments right now.. his insulin. his fluids. his anti nausea. the Mirtaz (I call it Mirtaz..). the potassium. to just stop everything else cause things are so gone.. the vet was trying to be as kind as possible.. they're like, give yourself and him a break. just make the days left really nice. the one other bright spot, if I chose, was to stop the Telmisartan to see if he perks up next day or two. and you know? he missed one dose not long ago and he seemed a bit perkier that day..... we can't win. yes hypertension bad, but this was the flip side, too. because the truth was that I was so wary about doing anything cause of that garbage Amlodipine and my feeling it was partially why we were here in the first place; pressure meds can hurt kidneys. too much can hurt. his pressure tests are unreliable. how to treat it for him? but I was encouraged all over to do so.. I understand why, but this is something I'M going to have to carry now.

all in all.... I think things are over, everyone. I can't lie that I want both our pain and all this agony to be over. I wanted my cat to stabilize, be ok, but its not looking that way. and he's got life left in him.. but his body is really hurting him. unless I hear otherwise maybe, from here, the other place (which I haven't heard much after I uploaded all the tests they wanted, too.. sigh.. they just drop off).. on something, or something maybe I should know.. I guess this where we at. this also wasn't the regular vet; its the weekend and they closed, but the one we been using ticked me off also. because of them holding the bloodwork hostage we started the Telmisartan; they also were the only one w the smaller renal panel.. I thought bloodwork affected anemia and that was better to do (the tech and vet I just saw again said no.. it doesn't....). I don't know anything anymore. I hear ten different bum opinions.

anyway.. he's down now to around 8 lbs, from 10 lbs just two weeks ago.. he can't really see anymore. but he's been doing ok, enough so.. everything else is the new normal, but the bloodwork... and now the weakness is pretty bad. all the binders I've been doing didn't work. I'm doing everything we were supposed to do. and he's so tough, he's still walking around despite how severe his anemia is. still pretty good at eating. we're tough over here.. but that's why he's probably sitting in the box (he's weak). everything was for nothing; just bought an extra month.. or two. pretty much the anemia is going to kill him. there's meds (to encourage the marrow right), but the side effects are bad (in their opinion). I indeed plan on stopping things (Telmisartan) and seeing how he does next day or two. They think.. it lies somewhere between April and August, probably got hypertensive, and then the kidneys were troubled, and then something w the Amlodipine.. idk. I'm debating whether to inform the vet we were using. I'm debating whether to stop these other things (his supplements, etc).. I think today I'll do essentials w how upsetting this all is. tmw idk.. I'd like to still do his Porus One but the vet thought it don't matter much it ain't working.. They said if we stopped all treatment for example, things would be over in a few days.....

of course in my upset I didn't realize I didn't leave with all copies of the bloodwork.. over $400 bucks too!! that's a racket right?! could go somewhere else and get it done for less than $300.. im not sure why they charging that..... but the weekend, and this is why we stopped going there. but I'm glad I did today. to talk to this vet. its just an opinion.. but I'm seeing this baby getting weaker. I don't want that for him.. I'm thinking I should think on something in the next few days. call this number.. apparently they had helped the vet and the tech themself w an ailing pet.

I'm so angry.. and so disappointed. my hearts broken; my soul is crushed.. we're exhausted. and w everything.. he's still cute tho. 💔 I feel this came so fast.. like a decision needs to be made immediately, now to avoid even more heartache.. he just tanked again, over the past 2 weeks.. sigh. I got nothin'.

Yasmin...sending you all the healing thoughts and the best good-decision-making vines I can as you care for Pudge. I can't imagine how difficult this is for you. What I do know is this: you have not failed your precious Pudge. You have been an ongoing champion and WONDERFUL advocate for him in his life and especially as his health has become so complicated--what a lucky little guy he is to have you by his side. I can only say I understand, and I am very sorry. It can happen sometimes that you do everything right in managing and juggling the issues, until they just get to a point where it’s not managing anymore. I went through that with Maverick, things came so fast and then I’d pivot, try and fix one thing, and then another thing and so on. Give yourself some time now just to be with him and love him. You don’t need to feel any pressure to make any decisions. Right now you are overwhelmed, and it is okay to feel that way :bighug:

I don’t know if you’ve seen this post or if it will help you. It’s helped me in the past. I found it helped me deal a bit more objectively with any decisions I needed to make whether it was to continue treating or stop.

Along with the others, I’m also sending light your way and peace of mind to know what to do, when to do it, and how to go on after.

One thing I want to encourage you is whatever decision you make and whenever you make it, try not to look back and think, “if I’d only done…..”. We all take the best care of our babies and he knows and feels your the love. He’s not judging you for anything you feel you should have done differently.

I agree with the above statements. You gave him a beautiful life and it's so very obvious how much you love him. And he knows it too ❤️

I've decided.. we're going to ride out the next 1.5 days.. til Monday when his current vet opens. Go from there.. or, see what happens in the interim til then. He got his fluids just now.. that should help some? I could see how much anemia drugs are w the one we just saw; but they weren't thrilled about them..

Will stop Telmisartan. For now..

Partly why this has been so scary in particular is juggling the FD.. and making sure he eats.. and the challenge of eating.. sigh. 💔

I'm looking at the anemia drugs.. Epogen apparently can make them really sick or make kidney issues worse if it goes sideways? (???) Darbapotein is better? Or the Varenzin-CA1.. which stimulates their kidneys themselves, no synthetic erythropoietin.. sp.... but do you have to have certain amount of good kidney left.. idk. Expensive? Idk.. if it buys us slightly more time so we're not frantic. His current vet spoke to to me about anemia drugs and wasn't put off by them.. so I'll see what they say. We'll go in.. if still around.. Monday. He's still got life in him.. just feels like absolute trash....... the vet stress also tanks his little body too. I hope he'll be all right.. idk. 😖
 
I saw your post on the tanyackd group and I replied there.

Thank you for the photos. Your little man is so beautiful. I would stop the Telmisartan and I would treat that anemia immediately. He should have probably received an Epogen injection. With an HCT that low it makes them very weak. It is worth a try. Or a transfusion. Don’t let the vets give up on him. It can make him feel better quickly.

Have you read the felinecrf page about crashing? This may not be the end. His phps needs to come down. When Darcy’s phosphorus was sky high and his kidneys were failing I had no choice but to feed prescription renal foods. It was most important and the carbs didn’t matter because I could adjust insulin.
 
This is the hardest part of being a pet caregiver. :bighug::bighug::bighug:. I too have used that post that Marje linked. You have done so much for Pudge to date. I hope that what you do this weekend will give you clarity on what to do next.
 
I definitely agree with Suzanne on feeding the renal prescription diet at this point. The phosphorus really has to come down. It can make a big difference in how he feels.

(?).. what do u think of Epogen?…. also tried looking for crashing page dont see it..

Unfortunately, while RAWZ is lower P, it’s not low P as a therapeutic diet is and that’s what you need.

When you say he can only have turkey because that’s all you know is “safe”, do you mean for his gut or some other reason?

I also missed the info on why he can’t take amlodopine? I had a CKD kitty on it for years with no issue but ECID. What is his BP without meds?

I hope your vet can come up with a BP med that will work and allow him to have an ESA. I haven’t heard anyone say before that Telmisartan could cause worsening creatinine. Have the members in TanyasCKD group found that with any of their cats?

Can he eat beef? Evangers has a low P beef. I believe it is in the 0.50% DMB range.

As you know, the nature of CKD is battling it on many fronts. IMHO, and I am only one, you prioritize what is going to make him sickest and treat what will make him feel better.

For example…..only an example and something to talk to your vet about, I’d, personally, use an ESA like Aranasp/darbopoetin to get the anemia under control because it is life-limiting. Keep in mind high P will make it a little harder to address the anemia. I’d also then use the Telm to address the BP because you don’t want him to stroke out. I’d get on top of the P with a therapeutic diet. If he can only eat turkey, I’d use the RAWZ with binder and I’d use the upper range that Dr. Nagode suggests (the charts are on Tanya’s on the page I linked). His creatinine is already really high but I’ve had a cat where it was higher (he had polycystic kidney disease which is the worst) and yet he didn’t feel bad because we had the other things under control.

With my CKD cats (and I’ve not lost any of them to CKD), I learned these are the most important things off the bat:
—phosphorus control
—preventing/combating anemia
—high blood pressure
—hydration

I’m continuing to keep you all in my thoughts. This is a tough disease on top of the diabetes.

It's so difficult to not think "what if" and also with all the concurrent things going on, there's no way for you to know how all the medicines and conditions overlap. You are doing everything and more to understand it for Pudge, making all the difficult decisions. I hope for the best for you two 🫂

I would definitely get Suzanne’s opinion or the CKD group because I haven’t had to deal with anemia associated with CKD. I’ve always started my CKD cats on B12 and a multiB when their HCT was 34% and none of them ever dropped below 28%.

Ok.. i been blaming myself about his B12. I finally got it and it has rice flour in it.. and no one on the kidney suggested to me something else. I tried giving but wasnt sure about his BG.. i know its partially my fault.

Well.. the vet we saw last night just called. They have Aranesp (Darbeopotein?) from a donation left in stock still…… he has to get a pressure test first tho.. but we have an appt at 2:30…. Wut u think?.. 🥲🥲🥲

Do we bother tho?.. i just read it could take weeks.. 😓😓😓
Darbo is slow to work. Epogen works more quickly which is why it’s used in cases of severe anemia where you can not wait weeks. Most vets stock it and can give an injection in house.
 
Ok.. i been blaming myself about his B12. I finally got it and it has rice flour in it.. and no one on the kidney suggested to me something else. I tried giving but wasnt sure about his BG.. i know its partially my fault.

Yeah.. hes been getting injections. But not the capsules.. I tried then the rice thing and i asked for alternative but it got away frm me.. my fault.
 
😪.. someone on kidney board just told me theres not much to be done w ESA’s for him cause they raise blood pressure.. smh. @Suzanne & Darcy 🥴😪
You should check that with your vet. I have not seen an increase in BP in my cat, but she didn’t start out with high BP. I read that they can raise BP but don’t necessarily have to. So best to check with the vet.

I am so sorry. So so sorry. This happening to Pudge has really upset me. I had a hard time sleeping last night thinking about you and Pudge. And I am sitting in the ER now with a cat.

If you have been doing the injections that is very good. They can even be absorbed better than oral B-12 because it bypasses the digestive tract. You have not harmed him because of the oral B-12!

oh Suzanne… 💔💔💔 i feel like its all my fault!!!!! i didnt do this didnt do that.. didnt see he was weaker. but i thought it was the phosphorus!!! 😖 we just had blood done two weeks ago.. i dont understand!!!! Just thank u so much…. For that and for for caring about us.. your advice and support has been so important.. i tried.. i really did….. i dont understand. 💔 im sorry youre at the ER w a kitty!!

i didnt get the oral B12 supplement in him.. everything else but that.. cuz of the rice flour i was worried about his sugar.. tried it and it seemes to elevate it but idk.. i asked for alternative on kidney.. heard nothing.. it got by me…. 😓😞 he didnt get his oral B12 much…. injectable yes…

And now he feels like extra garbage cause his sugars are high (for us).. that cane be good for his body rn.. i wanted to aSk here but time got away frm Me…… but thats what i heard to do.. Suzanne we cant win sometimes.. :( he wouldnt eat well in the morning.. so got half….. i just mess up. But i did it cause he isnt eating good.. 🥺

Suzanne no one has Epogen.. all Darbo or dont have anything.. gonna try one more place..
