6/19 - Oliver

This is Oliver's 5th day without insulin and I am pleased with his numbers. However, his BG seems to be creeping up a bit. His AMBG over the last few days has been:

60
66
88
89

Should I be concerned about this? Ar we headed to more insulin and, if so, at what point do I start?

That number is pretty perfect and still very much in the normal range....nothing to worry about here :mrgreen: !!!

A couple of weeks ago, I noticed that the pupil in one of Oliver's eyes was much larger than the other and didn't contract nearly as much as the other eye. I took him to the vet right away. I thought maybe it had something to do with his diabetes, but the vet said that was unlikely. She looked into his eye and said the nerves at the back of the eye looked good and she didn't see any evidence of a tumor. I will probably take him to a kitty ophthalmologist when I recover from my most recent vet bills.

I was wondering if anyone else has seen this is their diabetic kitty. When my sister was diagnosed with type 2 diabetes, her first symptom was double vision and a crossed eye. It seems logical to me that Oliver's eye problem is diabetes related, but maybe not. Just curious if anyone else has seen this.

I think a visit to the opthamologist is a really, really good idea. I would be sure they check the pressure in his eye. You might want to pop over to Cinco's condo or PM Tricia and ask her about his symptoms with his eyes because he was dx with glaucoma. No apparent reason why he got it. But I would definitely have it checked out sooner rather than later.

So he's doing ok without the pred? Great numbers!!!!

He seems to be doing fine without the prednisolone. He was due for it again tonight, but I have skipped it. Will give it to him tomorrow. I want to give his system as much time as possible to recover. My vet wants me to give him the pred. every other day for 2 weeks, then discontinue it. I figure, every third day is close to her instructions.

You need to follow her instructions. Pred is a drug that you have to very slowly taper. If you give it twice a day for a time period, then you give it once a day for the same period, then EOD for several days, then some vets will have you stop it or give it every third day for a few days and then stop it. So please be careful changing the vet's schedule with it.

Sorry but you really have to be careful with stopping steroids.

From Wedgewood Pharmacy (it's a great compounding pharmacy for animals):
Do not stop giving your pet prednisone or prednisolone abruptly; particularly if it has been receiving high doses or has been on the drug for a long period of time. This can cause serious, even life-threatening consequences. The dose must be tapered. Your veterinarian will advise you on how to slowly stop the medication.

Yes, he has been tapered from 5 mg/day to 2.5mg to 1.25mg over the last 3 months. Now he is on 1.25mg every other day. I just hate to give it to him since I can see how it makes his BG spike. But I will.
 
I know but it affects the adrenals and so there is a very specific reason behind tapering. I appreciate you being cautious and following your vets instructions. I don't think, at EOD, that it is really affecting his BG that much. He's still in great numbers. :-D

We've been watching Oliver's numbers without insulin and we think it's time to try an official OTJ trial!!! :-D :-D what do you think??

Here are the instructions:

  • Test at your normal AMPS and PMPS times. Feed multiple small meals throughout the day as much as possible (small meals are less likely to overwhelm a newly functioning pancreas).

  • If he is green at your normal test times, no need to test further until the next "PS" time, just feed small meals and go about your day. If he is blue, feed a small meal and test again after about 3 hours. If his number is lower 3-4 hours after a meal, then his pancreas is working!
  • Post every day so we can monitor your progress and see if any tweaks are needed. It is not unusual for cats to get lots of blue during their trial, especially at first. Don't panic and don't shoot unless we tell you to.
  • After 14 days of no insulin, we have a party!

Sometimes the trial doesn't work the first time and we have to give a little more support in the form of resuming insulin. It's not the end of the world if that happens, we just give him the support he needs. Our goal is a strong remission and it's better to take our time to get that than to rush into remission just to have it fail later on.
