OK, not starting with craziness. :thumbup

Hoping for something closer to normal today, I want to see my son and meet the girlfriend. Besides, I had planned to make a Turkey Pot Pie and take it to them today. That's been planned for 2 weeks, the bbq came up Friday. Yesterday was a big disappointment on so many levels. :sad:

Lucian, just stay calm today, no skyrockets, no scuba diving. @-)

There is a tactic which a very few folks have used to extend the duration of Lantus when they couldn't get home within 12 hours to test and shoot. It is dividing the dose into 2 parts, and giving the 2nd part 2 or so hours after the 1st part. The idea is that it spreads out the curve duration somewhat. Here are some posts on split dosing for you to review and see if that is an idea you might want to try.

I've heard the term and wonder if it would still help even keeping a 12 hr cycle?

If there was some kind of consistency with his numbers, it would be much easier to figure out a way to work with it. But there is none.

Predictably unpredictable.

With the mid cycle spikes and the fact that I have prevented him from earning a reduction, I'm wondering if he actually needs one?

I know rarely a cat with too high a dose will spike mid cycle and throw uber high numbers.....Lucian has never been by the book, so I wonder?

Will test in 30, when he hits 400, I go for the R. I had to give it a number, so I picked 400. I need specifics, to determine when to do something, that's just me. Thoughts?

Here is my take. Caveat emptor and all that because I may not have a clue as to what I'm talking about here, so please take this with a grain of salt. Or maybe an entire ocean full.

I'm not sure you have been keeping Lucian from earning a reduction. You've been doing intensive 'liver training school', trying to keep the BG's on a more even keel. Whether that has been by managing the food, or giving the R when he has gone high, the attempt is to flatten out the cycle.

Giving the R has not prevented him from going too low. It has prevented him from going too high. It is an attempt to get Lucian's liver to realize the swings in the BG's are ok. But to keep those swings from being too wild.

Big jump today between +6 and +8. My question would be, is it time to pull out the R when you see a big jump like that. More than basing the R dosing on a specific number? I have never used R as a bolus so I don't know if that is a possibility or dangerous. I think it's time to PM one of the R users and ask them to come visit your condo.

Thanks Deb, I don't know what I'm doing, I'm just trying. He dropped too quick today and now spiking. Just gave the R, but he'll be well over 500 before it kicks in, didn't know he was gonna jump that fast or I would have given it 30 mins a go. ohmygod_smile

Surprised how far it dropped in 45 min, he didn't make it to 500 thank goodness. He does everything quick, drop or spike.

Of course, 2 hrs after the R, he's climbing right back up. WTH am I supposed to do?

Only risen 8 points in 30 mins, hopefully he won't be too high at pmps.

I'm really wondering about the 3 times a day dosing, it seems his insulin isn't lasting 12 hrs. Has anyone else done that?

Maybe Levemir's longer duration would work better?

Hon, I have no way to get Lev. :sad: I have 3 full pens of Lantus, plus one that has about a third left, that I'm using. I've got to use what I have and what I can get.

Tell me more about dosing 3 times a day, I may have to try that. But wouldn't the dose, being smaller each time, make him even higher? Maybe just the first time?
 
Look at his SS for the last 3.5 hrs.....

+8.5 = 493 Gave R
+9.25 = 343
+10.5 = 412
+11 = 420
+12 = 350 PMPS


Makes no sense to me.

Take a look at my Insulin Dosing Options signature link to see some visual layouts of different tactics. You take the total daily dose, divide by 3 and give that amount every 8 hours. There is much more overlap, so when a spike starts, it quickly runs into the onset of the next insulin dose.
