? 5/26 TR Increase Tonight or go to 7th cycle

Am I jumping the gun under TR to increase at the 6th cycle or stay with this to a 7th cycle? He's seen his best numbers the past few cycles at this dose, but it's still not getting him under 200 (unless this is happening in the wee hours of the morning, but unlikely). This dose has also given him the longest flat cycle since starting TR a month ago. I am home all day tomorrow to test but not on Sunday.

I spoke to the vet and when we hit 5 u Jimmy will get a fructosamine test to see where we go from there. Which at this rate will be next week.
 
Yes, I would increase after six cycles if all you are seeing is above 200. Remember, the goal is eventually green.
 
I would also increase on the 7th cycle. It's great that the numbers are improving. That means you're on the right path. You don't want to lose momentum now by holding a dose too long.

Not sure what good a fructosamine test will do. Have you shared your SS with the vet? If he gets to 6 units (and we hope he won't), there are some high dose conditions he should be tested for.
 
I have - and she is aware of where he is with his dose. She actually doesn't want to go higher than five - why is 6 the magic number from this forum's perspective? I understand high dose tests would be for IAA and Acro - is this correct?

At six units, almost one or the other, or less likely Cushings, is present. Having said that, even though they are called high dose conditions, there have been kitties test positive for acromegaly on one unit. But we see more of them on higher doses. One in four cats has the condition.

Thanks - I'm hoping we're close to finding the happy dose, as his response to these higher units are promising. @Wendy when decreasing doses at these "higher" numbers - is it the same increments down as it was up (.25, .5, .75) or is it decreases by whole units...OR does it totally depend on Jimmy and his reactions? :blackeye:
 
We typically decrease by the same amount as we increase, ie. 0.5 units change when at 5+ units total dose. But this is where you have to learn what works for Jimmy and the circumstances may alter that decision. When Neko's IAA broke after her SRT (radiation therapy for her acromegaly) her insulin needs dropped very quickly. There were times I did back to back reductions and I did 0.5 unit reductions down to about 3 units dose. And that worked in those circumstances. Having said that, I have seen high dose cats be sensitive to smaller than normal changes. With higher dose we'll be cautious at first, because of the larger depot involved.
