Hi,

We're new here. Feeling a bit nervous! I managed to get my vet to change us to Lantus having been on caninsulin. :mrgreen:

My plan is to give caninsulin for PM shot as usual and start Lantus tomorrow at AM shot so I can be around to do a curve.

Is 0.5iu a good place to start?

For info: we feed wet food and Rhinog is on Prednisolone 2.5mg twice a day

Any thoughts or advice would be welcome.

Looking at your SS it looks like you almost had him off insulin, seems the neighbors dry food blew it for you guys. No problem though I don't think it will take long on Lantus before you get OTJ hopefully for good. Ok the Predinson does make a little problem but.....is it just a short term treatment?

I would say .5U looks like a good place to start, I'm thinking the prednisone is what's bumping the numbers up a little higher so he needs just enough help from the Lantus to overcome the effect of the steroids on the BG.

Tomorrow is a fine time to start especially if you will be around. Good Luck!

Just one thing......You got your Lantus and i assume you know how to store it, handle it etc. it is different than Caninsulin (no rolling, shaking etc.) do you have the correct syringes (U 100 insulin syringes with 1/2 unit markings)? You CANNOT use the syringes from a vet that are for Caninsulin ( U-40) you would be giving to high a dose. Just want to be safe here.

Yes we were OTJ for a year then the neighbours dry food along with the prednisolone meant the honeymoon was over.

Unfortunately the prednsiolone is permenant. It's working wonders on a lymphoma.

We have 0.3ml 100iu syringes with half unit marks. I've read all the stickies so no shaking or rolling!

Hi Ruth and Rhinog, Welcome to Lantus Land! I'm not a dosing expert, but just wanted to say hi and to tell you that you've come to the right place. Some very knowledgeable people will be along later today to give you some guidance. They will want to know what Rhinog's dose was on Caninsulin, how much he weighs, etc. etc. We follow a modified version of the Tilly Protocol. You can read about it and about all of the other things we follow (like diet) in the "Stickys": the information posts at the top of the Lantus forum. It's a pretty steep learning curve, and you will have to "unlearn" a lot of what you have done with caninsulin. Lantus is a great insulin, but it works very differently from Caninsulin and you will need to learn about this.

So, welcome, and ask as many questions as you need. Everyone here will help you to the best of their abilities. And as a support group, this is a great bunch of people.

Welcome aboard! I'm certainly not the one to be giving advice, I will leave that up to the experts... but I did want to say welcome to Lantus Land. You have found an awesome place, filled with very knowledgeable and caring people.

I already posted below, I think I'm knowledgeable? Anyway I found most of that information by looking at the spreadsheet. Everything except weight, clear was the fact that Rhinog was OTJ until recently which seems to be related to a dry food hijack and beginning predinsone. The current dose of Caninsulin is less than .5U (numbers are not exciting but still in the low yellow and a few blue, which on Caninsulin they shouldn't go below 100 anyway) So .5U would be a good starting place. the dose of Caninsulin a year ago wasn't very high either (under 2U max and then quickly down) and according to the SS in the beginning that was with dry food!

In addition I think I heard somewhere that lymphoma can reduce the insulin requirements.
 
Welcome to Lantus Land (LL).

It's great that you've done a great deal of the homework and already familiarized yourself with the stickies. In addition to your SS, it will be helpful if you could also complete a Profile. Since Rhinog has a medical condition and is on pred, it will be helpful to have a document with any other relevant medical history, what you're feeding, etc. You can attach the Profile to your signature so we don't pester you with the same questions repeatedly.

Since you're not planning on starting Lantus until tomorrow, I'd like to see if some of the other experienced users have any thoughts about a starting dose. You're seeing some good numbers on 0.4u of Caninsulin. I want to be sure that you don't start Rhinog on too much Lantus. Typically, we start low and work up to a good dose.
 
Hi Ruth, welcome!

I agree with Monique about the starting dose and 0.5u is the maximum I'd try at this point due to the reasons already stated; previous low dose of Caninsulin, was on honeymoon, no more dry food, etc. And as Sienne mentioned, you don't want to give too much to start, it is easier to work up the dosing ladder if necessary.

I bet it won't take much to get Rhinog back to green.
 
Welcome to Lantus Land! I hope your stay will be short (Rhinog is doing very, very well).

I agree with the others, I would start at 0.5 or even less (0.25).
