Words can not explain to you right now how it makes me feel to have such wonderful and caring people in our corner letting me know that Da***t, I love my Tommy and I'm not letting him down. I'm doing the best i can..and i won't let him or any other vet make me feel that way ever again!!
Wow.. I am exhausted, any one else feel that emotional drain after a stressful day with butting heads with your vet..

Hi Squirrel, Tommy is your kitty and you have a right to decline mediations that have been proven harmful to cats. I agree with the others, wait for the results of the C&S, get a prescription for a safe antibiotic, and find another vet! Isn't it incredible that so many vets just want to be dictators, especially those who have been in practice for a long time and just keep doing what they've been doing? They think that one size fits all--one procedure for everyone.

Anyway, We are sending lots of healing vines to Tommy and hope that he will be better soon.

Ella & Rusty

IMHO, vets, as are all of us, are entitled to their opinions. However, those opinions are only credible when backed up by research and fact and not what "feels" right. If your vet chooses to prescribe Metacam, I seriously hope his malpractice insurance is paid up and that he doesn't want to risk his veterinary license. If he went up in front of your state board for professional licensing, there's a good chance he'd lose his license for ignoring an FDA warning because he "doesn't trust the FDA." And while there's no way that the loss of a beloved pet can be compensated, I bet there are a few attorneys on this board who would love to help someone collect damages against a vet who uses such poor judgment. I would suggest that if your vet doesn't trust the FDA, would he want his pregnant wife to take thalidomide (black box warning due to birth defects like flippers instead of arms) or dietheylstilbesterol (DES) (lots of long term effects on exposed fetuses) or his child to be prescribed an SSRI (black box warning due to the increase in suicide potential). Prescribing practices have radically changed due to the FDA's intervention. Maybe he should take a COX-2 inhibitor like Celebrex for arthritis and hope he doesn't have a heart attack.

As far as Somogyi, the best that can be said about the phenomenon is that it is controversial. The original research was published in 1938 with a small sample of human subjects. Despite attempts, the results have never been replicated. There is a good summary in Wiki. The bottom line is there is no consistent research to support the phenomenon in humans. There is no research to support the existence of Somogyi in cats, let alone cats that are being prescribed a long-acting insulin such as Lantus. The definition of Somogyi presumes a chronic state of rebound. The bounces that we see are not chronic. They clear within 72 hours. Personally, I find it fascinating how many vets see a bounce and immediately conclude "Somogyi." I've done two literature searches within the last year in both medical and veterinary journal and associated resources and can find nothing other than anecdotal reports -- there is no scientific research. Ask your vet to provide you with the citations from the research literature and not to quote urban legend.

As for the credentials of the people here, while very few people come out and cite their pedigrees, this is a pretty good cross section of talent. There are MDs, nurses, PhDs, lawyers, teachers, epidemiologists, students and a ton of people with good common sense and great hearts. And most of them are intellectually curious, committed to the health and well-being of their cats, and willing to put the work in to keep their cats healthy. While not everyone here is an endocrinologist, the willingness to learn is nothing to sneeze at.

WOOOOOOOOOHOOOOOOOOOOO!! Sienne, would i have loved to have had you by my side today.. I am not much of a speaker, but i sure can spot one.. Very very well said and so eloquently put! I will be bookmarking this day!
On a much brighter note, Tommy seems to be feeling well, he went a nice pee pee earlier, tested negative for ketones, had some supper, his shot and over all i would say his numbers are pretty darn acceptable given the day he's had.. I'm so proud of him.. He was an awesome kitty today!!
I would just like to thank everyone again for the overwhelming support and encouragement that you gave me today.. It will NOT soon be forgotten :-D
Sleep Well My LL Family I-)
