5/19 Kabaret : Switching from Caninsulin to Lantus

Hi everyone,

I wrote a post last week about a cat whom I am helping that desperately needs to be witched from Caninsulin to lantus. We needed to wait an extra week cuz Sylvie (the cat's owner) had tu struggle a little bit with kabaret's personnal choice in termes of canned food!

We are now ready. She has plenty of a low carb food that Kabaret likes (fingers crossed that he will not change his mind).

Here's the little description I already posted about Kabaret :
The cat's name is Kabaret, he is 12 years old and has been diagnosed with diabetes on March 19th. Since he was eating dry kibbles we suspect the cat had black values ever since that day :(
Kabaret is tested for ketones every single day. Luckily, the results are always negative.
He weights 16 pounds.
The owner is Sylvie. She lives in Paris and doesn't speak or understand English.

Last time I asked you guys about the starting dose, 2.5U was suggested by Sienne & Gabby and julie & punkin.

:?: Looking at Kabaret's spreadsheet with all the new data (and the instability of his recent results), do yo guys agree that 2.5 WOULD STILL be a wise choice to start Lantus?

We will start Lantus in exactly 12 hours. So a confirmation before that would be very appreciated!

Here is the link to the previous post on Kabaret so people can see what was discussed. Will Sylvie be able to test fairly frequently the next few days? She's done a good job so far during the day and has a before bed test each night but just wondering how much she can monitor tonight and whether tomorrow morning might be a better start. Of course, with a depot insulin she may not see action right away anyway.

From the protocol sticky:
Kitty should be monitored closely the first three days when starting Lantus or Levemir.
Blood glucose levels should at least be checked at pre-shot, +3, +6, and +9.
More monitoring may be needed.
 
Hi Kim!

I'm still ok with the 2.5u as well. my thinking is that it's a lot easier to increase to a dose if needed than it is if we start Kabaret too high. We rarely start with the perfect dose - part of following Tight Regulation is that you're tinkering with the dose every 3-5 days anyway to get the cat into good numbers.

We'll look forward to hearing how the first few days go! After the first 5 days, the dose should be re-evaluated often, so keep posting!
