Yesterday's Condo

Morning - Well yesterday was quite a day with taking house sister, Miss Nubbins, to emergency vet just a Lanky's #'s started to fall. Having a timed feeder & having practiced with it helped immensely. Also, having a try of food all marked with the % carbs ranging from 0% to 18% proved to be valuable as I hopped around in a semi-panic. Miss Nubbins is much improved today, moving slow, but an order of magnitude better. Lanky happily ate Fancy Feast 3%. His coat is absolutely luxurious now. It feels soft & thick, I can hardly stop petting & hugging on him. His coat used to be more coarse & thinner.

Sure hope we get another round of good numbers & a pass on emergencies today. Just followed Lanky to LB, got negative ketone read, other business was fine too.

PS I did not get up in time to shoot +11.5 so +12 was done.

Great. You didn't "need" to shoot a half hour early, I was giving it as an option since number was high.
I don't think we'll be returning to any around the clock type shootings. One round was good.

I just did a test, +4 262 & gave Lanky a small FFtb 3% snack. Now I have to run to Kaiser urgent care for my husband for test kit & medicine - him medical emergency is likely infectious diarrhea. Can hardly believe the weekend I am have. This drive is about 20 miles further than the vet from yesterday. Hope to be back in about 2 hours.

I'm back & I have to make another run to Kaiser to return the samples later this PM or tomorrow. Do you think I missed Lanky's nadir or did he stay in the yellow? It is overcast, 80 degrees and steamy here. Lanky is slow moving & so are the other kitties.

I think he stayed in the yellow. His bounces are clearing faster so next cycle might better.

this sounds like one of those weekends where getting back to work will actually be a relief. I hope it gets better. Lanky is doing great, though!

I am back again. I'll test Lanky @ +11. He is feeling much better after his thro-up episode. In fact, he's sitting on the counter trying to encourage me to feed him. But he'll have to wait until after PMPS.

Not sure what tonight's game plan should be, as I have hardly had time to think today. Let's start thinking & then the +11 should guide us.

You have really had one heckuva weekend. I hope you can sit back with a glass of wine AND chocolate 'cause you certainly deserve it.

I'm guessing that they did not do a culture on Ms. Nubbins. You may need to keep an eye on the UTI and get a cytocentesis (sterile urine sample) once the effects of the ABs are out of her system.
 
Do you know what AB they injected yesterday? It it was Convenia, it lasts for 2 weeks. If it was something else, it may be shorter acting but it may effect the outcome of any culture and sensitivity (C & S) that the vet runs.

Sienne - Miss N's injection was Cinvenia Inj X 3.6mg, straight from my receipt. The emergency vet told me this was used in Europe, still relatively new in States & especially for UTI. Miss Nubbins, the sweetheart that she is, is ABSOLUTELY impossible to pill or give oral liquids, She's a foamer at the mouth. Breaks my heart and then she (& my husband) won't talk to me for a week.

I see the usual pattern, Lanky's dose gets raised, he responds for a cycle or two and then back to the high yellows and pink.
I think there is a duration issue, which is one of several reasons, I like shooting early. I think switching to Levemir would be worth trying.
Lanky may get better duration and have less insulin resistance with Lev.
For this morning how about raising the dose to 6.75u?

That certainly sums up the pattern. I just got +10 (@ 4:15 am) of 407, ugh. Does that change the dose increase?

Personally, I'd rather it be 0.5u.
This advice may be considered to be aggressive in dose increasing.
I've thought about that, 1 out of 6 cycles was good on 6.5u, your testing availability, the duration issues, and most importantly the pattern I see with Lanky.

If this was Noisy, I would raise by 0.5u, then switch to Lev and see if it helped with duration. I'd also start thinking about an IAA and acro test. I have been told there is a trend here, and on the German forum, for cats coming in needing higher doses so I'm not saying Lanky is likely IAA or acro as much as I am saying I would want to remove it as a possible factor in our thinking. With Noisy I tried to remove as many (possible) factors so that I could have a clearer picture. Things like using a new vial of insulin every 28 days -- I don't know if it would have lasted longer but I sure didn't want to waste my time and his time wondering about it. Speaking of which how is your insulin now? If it is older than 28 days then I wouldn't go up 0.5u I'd open a new bottle and see what he did with that at 6.5u.

My gut says 0.5u increase.

Also, My regular vet will be in Tuesday & I'll be talking with him about Miss Nubbins. I'll take the opportunity to update him on Lanky & discuss where I'd like to go. So I want to get clearer & specific on what I want to say. We've spoken about ultra sound (pancreas area?), IAA & Acro test (IGF-1?), and different insulin. Will you start thinking about all this & help me get my vet request in order?? I don't believe they use Levimur but maybe they'll write a prescription if we want go that way.

Also, think about this throwing up episodes. Im going back in my notes on that one. Sub vet at last appointment commented that because of the throwing up something could be going on with Lanky's pancreas that manifests as FD (not sure if I remember his words/thinking accurately) & that was one of the reasons he mentioned doing an ultra sound.

Insulin is 18 days old - & I've been very careful with its use. I note days old on SS just to keep it on my mind.

I like eliminating factors so we are thinking in the same direction. Got my last post out before I saw all of your thinking...

So after AMPS, shooting 7.0u, and posting my new 5/17 condo, I am going to start making my vet list. I'll post it so you can look it over.
 
I just sent you a PM -- hoping you see this before you shoot. There are concerns, from others about raising the dose this morning. You should hear them out.
