Rosie recently spent 3 days in the hospital for treatment of severe constipation. Unexpected hospitalization disrupted her insulin schedule and dosage. I skipped one shot and noted NS in her spreadsheet because of lower numbers when I was able to test. I am aware of the general guidance regarding reductions and .25 increments but I took more extreme reductions to be safe because I could not monitor / feed during these 3 days (5/11 - 5/13).

In the hospital, Rosie received significant fluids, multiple enemas and some "massage" to encourage progress. Pumpkin has been discontinued. Mira-lax has been added, but ongoing monitoring will be required to avoid a re-occurrence. Rosie is home now and I think she is starting to feel better. Her appetite is good. The reason for my question today is she is responding well to a much lower insulin dose. Requesting spreadsheet review from experienced folks to help me understand:
  • Is this depot drain from previous dose of 4.25?
  • Should I start over following TR guidelines (with current dose of 2.0)?
  • Did she have increasing insulin requirements due to constipation (over the last month or so)?
Previous.

Did she have increasing insulin requirements due to constipation (over the last month or so)?
I would say it is entirely likely that the constipation was causing the higher numbers. She's looking really good on this dose, so if she were my cat, I'd keep her here until she tells me to do otherwise!

Is this depot drain from previous dose of 4.25?
Maybe at first, but the depot would be completely readjusted by now.
Should I start over following TR guidelines (with current dose of 2.0)?
Not sure what you mean by "start over". If you mean, start the count again, then yes. The count restarts any time you change the dose or skip a shot. She's only had three consecutive shots at this dose right now and she looks good. I would hold it for a total of 6-10 total cycles.

Sending good poo vines!

I agree with Tricia. I'd stick with this dose for the remaining 3 cycles and evaluate at that point since you're following TR. I'm glad to hear Rosie's feeling better.

FYI - with Miralax, remember to add water to Rosie's food. Miralax pulls water from other areas of the body into the gut so keeping Rosie well hydrated is important.
