I am keeping Silver in my good thoughts and prayers. I hope your vet is taking him home tonight and is doing some mid-cycle testing. Keep up the trust in your vet and keep up the confidence. I know how I felt when J.D. was hospitalized with DKA. I hope you get some good deep sleep tonight.

Called vet. He said Silver's potassium levels are slowly coming up but he can't understand why his BG isn't coming down so he upped his dose AGAIN!

I went through this with my previous feline diabetic, Stu. We had to switch to Lantus from one of the older insulins that was discontinued. The vet followed the old rules and raised Stu's dose frequently by 1 Unit at a time. Blood glucose numbers kept going up for several weeks (we were up to 7 units) until one day when poor Stu crashed. That was when I found the FDMB.

Now, Stu did not have DKA, and your vet may well be basing his dosing on multiple health issues--not just diabetes--, but I think you were right to bring up the fact that a depot insulin, like Lantus, has to have several days to settle on a dose before making changes. The addition of a fast-acting insulin to bring down the bg sounds sensible and perhaps the vet will be willing to try that approach. The people here who have used a bolus insulin along with Lantus or Lev had to be very careful when they administered each so as to be sure that the two insulins had the proper distance between when each was shot. @Wendy&Neko would be able to explain this better than I.

I hope that Silver will be OK at the vet's tonight and I hope that you will get some good sleep.

p.s. I see that the May page of the FDMB calendar has Silver's picture. I consider this a good omen.

Juliet, my advice is ask questions. By asking questions you are accomplishing 2 things. One is that you may learn from the doc, as he may indeed have a strategy that you are unaware of.

Secondly, you can make sure that he has thought through and has a reasonable strategy that he can explain. If you feel he is talking over you (and he may be, some vets are better at communicating than others), perhaps email is a better way.

I would pose a question like this: "can you explain why you are doing X, as my understanding is Y?" Make sure you state in your email things like "I'm just trying to make sure I'm following/understanding" and/or "I want to work as a team to get Silver better" etc.

Yes, you have to trust, but also be aware of the treatment plan and the logic behind it. Maybe this is not your style or what you want, but I thought I'd put it out there.

He days he can't understand why Silver's BG isn't coming down and doesn't listen to me when I explain that Silver always goes up a bit after a new dose; is stressed out so it's also high and it's actually lower than he has been in several weeks so I'm happy with his BG.

I'm sorry he doesn't seem to be listening to you. You could also ask if he has consulted any specialists or researched online to get help with the answers he doesn't know. May help, may not.

ETA: Make sure your questions are specifically about his treatment plans and not just about Silver's response/lack thereof.
