Hello darlings,

Hope you and your kitties are all doing well. Me and my Barsa are confused. Not that long ago we were up all night drinking water and testing blood/urine to flush out ketones. Now we are up all night Again, but this time because of low sugar. What a rollercoaster! It's going to be my second night of tight monitoring today. Nothing but green numbers. Don't mean to boast but have a look at my spreadsheet. I am glad and at the same time scared! In hindsight I probably should have reduced the dose but I was expecting higher numbers today. Today I tested couple of times in parallel with my other glucometer. The numbers on the spreadsheet are from Accu Chek Aviva. So 3.4 (61) @ +2 is 2.6 (47) on One Touch Ultra 2 meter, 3.1 (56) @ +3 is 2.4 (42). So she's actually even lower. And it's not even close to +6 +7 yet.. Definitely a reduction. Agree?
 
Oh my, who is this cat and who switched her with the Barsa from before? :D:D:D 24 hour in green, exhausting but amazing! I'm also celebrating a week without ketones. :)

Typically we suggest just sticking to one glucometer, because switching back and forth can drive you crazy. We do have people here use the Aviva with success. The standard advice would be to stick with one meter for the cycle, see what the cycle looks like and whether she earns a reduction. You did give her high carb food already though, so that will influence the rest of the cycle. However, Neko's vet uses the One Touch Ultra and has done a lot of favorable comparisons with lab results. And that 2.4 (42) is low 40's. In my opinion, I think you'd be OK taking the .25U reduction. Just keep an eye on her, and if Barsa doesn't get back into green numbers on the lower dose (after any bounce is gone), then I'd go back up. And keep testing those ketones as you have been.
 
@Wendy&Neko Thank you for replying! I am really glad to be sharing these good news today :) It just feels Amazing after such a long struggle to see these numbers. I am sticking to Aviva now but because OTU2 is more accurate I was curious what the reading will be. Oh I wish it didn't require so much blood, it's a really good meter.
 
Wow - look at the change in her numbers from just 2 days ago until now. You must be exhausted, but those numbers are so worth it! :D :D
 
With max, if I don't hold for a day or two past the low numbers, he starts to go up again, but I also was checking him a lot when adjusting the dose. You want to stay in the green numbers. Once max reached the dose that puts him in the greens, each day after that he would go a little lower.

When you do reduce the dose, check daily again for ketones, as they can reappear with reductions.

Also, with levemir, it seems we are either 300-400s or 60-80s, like flipping a light switch. So if you see high numbers after the reduction, hold the higher dose a few more days.
 
@Meya14 So here's what happened since last night. Because she went down to 61 @ +2 and the other meter showed even lower - 47, I decided to give some high carb food because it looked like she'd go even lower by +6 or +7. But because it was actually my first time dealing with low numbers, I gave 2tsps not 1, which maybe was too much. Then an hour later another teaspoon. Then at +7 I tested and bg was 140 so I realised those 3tsps were more than enough and numbers will be high at AMPS. Went to bed. Mornings AMPS - 391. Gave her the usual dose of 3.5units. The idea was to lower the sugar to blue numbers or so and then in the evening give a reduced dose. Something like that. BUT while I was taking a nap, she escaped outside in the garden (someone in the family wasn't careful) and most likely got into our neighbour's house through just installed cat flap and ate their food. Great! She's looking very good, very happy and her BG @ +4 is 583. Nice..
 
It might take a while to get back down again so I'd stick at the higher dose for now. The higher her numbers are, the less the pancreas is able to kick out it's own insulin which makes things hard.

I feed my other cat half dry food (to maintain her weight - she's 16), and once in a while, Max will sneek in and scarf all hers, and all his wet food. It happens. Once he is good and in the greens, this extra carbs doesn't raise him much because his pancreas can deal with it. Prior to getting him in the green, even a spoonful would raise him by a lot. That's where you are at right now. I know it's hard being right on the edge of too low, but for us, that's the only way to get his pancreas to start handling things. I try to hold his numbers really low, after about 2 weeks in low numbers, the food spikes really don't happen as much.

That being said, it's good that you fed him the high carb, and you were able to see how the sugars react. I hear a lot of people saying that high carb food wears off fast, but that's not what happens with max and a food spike. It raises and can stay up for the rest of the day, so a little goes a long way.


Also, I find that giving a spoonful of higher carb early in the day is better than waiting for a low nadir when the insulin is going to start wearing off. I'd usually give him a little if his numbers were in the 60s-70s an hour or two after shot time, then he surfs all day on good numbers.

I would say that today's numbers are a bounce from all the green yesterday and the evening before, but of course if you know she got into some food that she shouldn't have had that might be playing a part as well.

I have to think of some kind of solution now that neighbours got themselves a cat flap. I can't listen to cat concerto all day long, it's just not possible to keep her indoors all the time.
 
I'd watch her carefully if you think a lot of the high numbers are due to carbs. I've seen Neko go from black (32) to green (3.8) 18 hours later after some bread starter worked it's way out of her system. If that happens, you might not want to wait too long before taking a reduction.

Cat concerto - love it!!! I`m taking mine to the vet this aft so will be cat concerto for me on the car ride..:p
