5/03 Cloud PMPS 138 (AT2) not sure whether / how much to shoot

Cloud seems intent on spiking my blood pressure lately.

Last 24 hrs he seems to be coming off his bounce. Today:

AMPS BG 304 (AT2 meter) at 6:53am, normal 3.75U insulin at 7:02am.

He had eaten about 2tsp of diabetic dry food (Royal Canin DM) sometime in the night, felt a little dehydrated this morning and only ate an ounce of wet food (Nutra Max Cat Chicken & Liver pate, not entirely low carb). Did not feel like he had a fever (but previous day he felt a little warm).

When I got home from work, an hour later than normal because I had to stop and buy syringes and cat food, there was evidence he had eaten another 2tsp diabetic dry food in the afternoon. Husband said Cloud refused the wet food he was offered at noon. Also said he appeared to be limping on right front paw - we think he gets his claws stuck in the fabric getting up on our couch and occasionally hurts his toes - he had the same problem with left front paw last week and it went away in 2-3 days. We inspected his paw (both times) and could not find an injury.

Could not find him anywhere this evening until an hour after normal shot time; new hiding place under end table next to couch.

When I found him he hissed at me (unusual) but came out. I may have surprised him. Fed him Tiki Cat Tilapia which is low carb food (he ate an ounce). May still eat a bit more. Checking for dehydration he feels less so than this morning - his scruff goes back in place immediately. Nose is moist.

There is a little over a 2-inch poo in the litter box and he peed at least 2x today, a reasonable amount, not a ton, not too little.

PMPS BG is [incorrect 183] 138 at an hour later than normal shot time. Unsure whether to give full 3.75 units since he is much lower than he has been.

If anyone is able to give some advice this evening I will gladly take it. Scared to shoot. Scared to wait to shoot.

You've shot a 118 and a 150 before. Can you monitor him tonight if need be? Do you have plenty of tests strips and high carb food? Another option is a reduced dose if you feel uncomfortable shooting this.

If you shoot, you can always get back on schedule by shooting 1/2 hour earlier once a day, or 15 minutes earlier a cycle.
 
Can you stay up and test if he drops too low? Are you worried he won't eat? How much is he eating? It's important that he eat enough or he could get fatty liver disease.

Wendy answered while I was writing but I still wonder what's going on with his appetite.
 
@Wendy&Neko Thanks for your reply. I can monitor a few times in the night (if he doesn't find a new hiding place I don't know about!) But I shot at 118 and 150 before I fully understood I should be watching out for how low he drops at nadir, not the values he's at pre-shot. I don't want to whammy him too low and start yet another bounce. I'll try shooting now since it's an hour late, and check on him as often as possible through the night.

@tiffmaxee He is eating about 4.5oz of food per day, which is up from maybe 3oz per day last week. But normal for him is more like 6oz per day. But tonight is his first really low-carb meal. Hoping he continues to like the food.
 
Man, he is loving this tilapia, and its really moist chow down, Cloudy! He may make it through a whole 3oz.
 
Also, @Wendy&Neko, yes we have some dry purina cat chow, which Cloud loves to eat, and husband insists on keeping around for outdoor cats. And I have Karo (corn) syrup in my pantry, just placed it on the counter in case I need it during the night. Also have lots of test strips.
 
If you can monitor, you should be OK shooting. If you are nervous, you could do a 1/2 dose. Tomorrow morning you'll need to shoot 12 hours later - if that works for you. In future, you might want to get some high carb wet food. It is processed faster in their system and is good to have around if kitty is low and you want their numbers up quickly.

If you get a +2 test, it is a good one. If it's about the same as the preshot, you'll have a normal Lantus cycle (smile shaped curve). A lower number means you'll potentially need to do more monitoring. A higher number means a quiet night.
 
Thanks again @Wendy&Neko - I will monitor as best I can. Right now he is contentedly purring on couch next to me. I'm waiting another 1/2 hr to do a +2 and then will try to get a little sleep.

Thanks for your help - so common sense yet I wasn't seeing it due to being in panic mode over his hiding from us. He rarely ever does that, and my first thought when I can't find him is that he will die with me not knowing where he is until I smell decay - and then I can't think properly. I need to teach myself to chill.

You're a totally normal sugarcat parent!! I think we all have those kinds of thoughts. I know my heart races when I have to leave the house until I walk in the door and see China is still breathing!
 
Oh geez, just rechecked my meter and realized his PMPS was 138, not 183. :facepalm:

But his +2 was 171, so I feel better. Will monitor during the night.

Added correction to my initial post and post title re: the PMPS BG level.
 
So last night at +6 he was at BG 201, this morning at +10 BG 279. Looks like it was the right thing to give the full dose. Phew. He ate last 1oz of Tiki Cat Tilapia at 2:00am, this morning had 1oz of Tiki Cat Chicken/Turkey. Husband will have to give the insulin dose and offer more food this morning since I have to leave in a few minutes.
