I would encourage you to do a curve before raising her dose again. That would make a 3rd increase in a week and I think that is too fast.

When I see a +7 of 80 after a PS of 428 and a dose of .1u just the day before you wrote her appetite was good, I just wonder if the 300-400s you are seeing is from not enough insulin.

ETA: That was poorly worded. Obviously you think she needs more from the high numbers and individual spot checks that are also high, but "I wonder...." if that is the case that they are "...from not enough insulin". I suspect she is getting too much, and/or the dose hasn't settled.

And what happened to all the tests? We have a pretty good picture of how she was doing a month ago and it's what led you to see she needed/needs less insulin. For the past 3 weeks we don't have that good of picture and important data may be missing.

I think if you can get more tests in, you might catch the lows which are precipitating some of these 300s/400s.

So like Sheila said, attempt to get a curve before you do another increase. I'm concerned an increase isn't needed. And I think Sheila meant the 300s/400s are from too much insulin.

I think, just slow down a little and make "considered" dose changes. She has a lot going on with all the meds, etc. so that's another reason to make more gradual changes. If you are going out of town, I would don't increase right before leaving. But lets look at the curve. Yes, every 2 hrs. is best. I know that will annoy her given all the rest she is "putting up with", but it is for the best.

I don't think she needs more insulin if she is getting down to 100. I mean, she isn't sitting in high flat numbers.

You could do one of two things at this point: hold this dose and watch for is for her to settle even more and stop throwing 400s out there. But she may not (settle) and it may mean she needs a bit of a decrease.

You could also drop her to .4u for the time that you are away. That would let her settle on that dose and you could do a curve again when you get back.

Really glad we could talk you into that curve before you raised her dose again.
