I started Gizzy on the Onsior tonight for his arthritis/joint pain. Either that kicked in and his pain is better or he's really responding to the 2.5u!! 73 at +5.5 (down from 406 at PMPS - WOW!) I can't imagine how these huge swings make him feel. Poor guy! I can tell he feels like crap again today.

The new vet I went to yesterday suggested that maybe Gizzy is a cat that only needs once a day dosing. She thinks that his insulin is lasting too long and the overlap is causing the extreme bounces. She said to keep him on 2.5u twice a day for now - especially since we introduced the pain meds and observe for a bit longer. We're watching to see if his #'s are still dropping at PMPS, if they are, then we either need to start a longer dosing cycle or try once a day dosing to see if that would help even him out and avoid the extreme bouncing. The next few days should be interesting....

I am very nervous to have him on this new med especially since it hasn't been approved in the US yet...but they have been using it in Europe for years with good results. I'm also very excited about it because if it helps with the pain - then great for Giz! I figure the quality of life living in pain is no good either - so why not give it a try? I'm adding extra water to his food to keep him well hydrated. I hope to see some results from him tomorrow - we'll see. Fingers and paws crossed!

Cats do not need insulin once a day; their metabolism is not like humans.
The big drops would be due to the dose needs to be lowered.
If the shot was lasting longer than 12hours, you would not see a rise to the next shot time.

You may want to ask the vet why you are to continue the dose of 2.5u BID if the vet thinks it should be given only once a day. I don't see the logic.

Once a day will not give you good numbers which you will see if you try it.
 
Strange that Dr. Myer would say that Gizzy needs insulin once a day. What is needed is less insulin, not going to only once a day. I would never go to only once a day Lori. Think about decreasing the dose instead. lantus will not last 24 hours in the body.

I can't find where onsior is to be given long term.
Is Gizzy getting this pain med for just a short time?

The dose of ONSIOR (robenacoxib) tablets is 0.45 mg/lb (1 mg/kg) orally once daily, for a maximum of three days.
Preoperatively: Administer dose approximately 30 minutes prior to surgery.
Postoperatively: Tablets may be given with or without food. See dosing chart for dosage directions. Dosing Directions: to be used in cats > 6 months of age and ≥ 5.5 lbs. Tablets are not scored and should not be broken.

According to Novartis, multiple safety studies were conducted on Onsior in cats, including a 21-day, 42-day and six-month study at up to 10 times the daily dose approved for use. Some cats experienced mild and transient diarrhea, soft feces and vomiting, the company reports. At higher doses of Onsior, vomiting, neurologic signs, intestinal ulceration, increased liver enzymes and kidney damage were noted.

Be sure to get diff pain meds like maybe gabapentin or tramadol if you need something other than bupe.
and also something for nausea, and for sure can you get sub! fluids for Gizzy?

Lori,
I do not know if you will see this. PLEASE have them do a renal panel. If Gizzy was having side effects from the onsior Kidney functioning MUST be checked.

Keeping you guys in my thoughts today.
