4/24 Darcy PMPS 158 Update

Darcy is looking and feeling soooo much better this morning. He actually got up and came into the kitchen and was MEOWING for food. He excitedly at half a can of Weruva BFF Play Lamb food (okay, with pure bites crumbled on top... he's still spoiled... hungry... but spoiled.)

Oh, and last night he had a good cycle of comfortable low-end blue numbers (as soon as he came down from his vet spike and late shot.)

We're off to the vet now to drop him off for his IV fluids.

Thanks to all of you who wrote with well wishes. It really means a lot.

Hi Suzanne, so happy to see your post and that Darcy is feeling better this morning. :cat:

Suzanne, SO glad to hear Darcy was feeling better this morning and hopefully even more so after the vet fills his tank today. Amazing how your perspective changes when you see them eating, isn't it? Minnie wants to know how Darcy feels about singing a "Hungry Eyes" duet at the next karaoke? I seem to remember the parties are in blue so Minnie still has some work to do to get her name on the list....

Hope you are doing ok, yourself. Also thank you for sharing wisdom on the fluids.
 
P.M. Update. After I got him home and settled and fed... I took a nice long nap!

I took Darcy to the vet this morning. Arrived at 7:30 a.m., which is before they open, in order to get the IV fluids started a little early (since they closed at 1 pm today.) I walked into the vet with Darcy because the door was unlocked (I called on the phone first, but of course, since they really weren't open yet there was no answer). It was all dark inside and I was calling out... hello... hello... I started flipping on light switches and nothing happened... quite dark. I set Darcy's carrier down and started walking in the dark through the vet hospital calling hello... hello. Tried more light switches in various rooms and still no light. I started walking toward the back where I heard dogs barking. I walked into the grooming area and found a woman I know (fortunately, I pretty much know all the staff there and they know me well.) She was working back there with a little light coming through the windows. I asked where the vet techs were, and she said they were "out back smoking." She explained that they had no electricity. I said I was surprised that they did not have a backup generator. She said she'd tell them I was there. The vet techs said that they could not do anything with Darcy because they did not have electricity so their IV pump would not work. They suggested I take him back to the car and wait to see if the electricity came back on. I did, knowing the vet would be arriving soon. She arrived at 8:15 and sent them out to take Darcy so that she could take a look at him. After we waited around for a while, one of the vet techs came out and said that the power company told them that they expected the power to be restored by 10:45 a.m! They actually did not get electricity back until 11 a.m. The vet said she could put him on a "pediatric drip" with no power, so I'm imagining that it simply an IV bag of fluids hanging on an IV pole and dripping in by gravity (the old fashioned way.) So we left to pick him up at 1:00.

The vet says that she thinks he looks a little better (I think he looks and acts a lot better). She said she had combed through the literature to see if there were any reports of kidney damage due to Cabergoline, and she found none. She did not repeat bloodwork today (they couldn't have anyway until the power was restored.) The plan is to bring him back Monday morning and keep him on IV fluids again for another day. I do not know if she will repeat bloodwork then or perhaps wait another day? I think she will base that on her examination of Darcy. I will give him sub-q fluids tomorrow. He still has his IV catheter in his arm. I need to go flush it now as a matter of fact. Because his blood pressure was elevated - around 168 - on Friday, she started him on Benazepril, which she said is also beneficial for the kidneys. She will re-check his BP on Monday and if it's not lower, we will move to Amlodipine. He just ate some more for me at PMPS (with phosphate binder). I gave him 1/2 an ondansetron mini melt in case he is nauseated (which he probably is due to elevated BUN).

Well, that's my long story for today (had to include the fun with no electricity). I have that Jury Summons on Monday morning and have to be at the courthouse Monday morning at 8:30. This means, of course, that I will have to drop him off at 7:30 (again before they are open, but the vet techs and others will be there). I hope everything goes more smoothly than it did this morning.

Sure. Darcy likes "Hungry Eyes!" (Thanks, now that song is in my mind... wasn't that from the movie "Dirty Dancing" with Patrick Swayze? Long time ago. I see Minnie went flying off to the moon at +4. I hope she came back down. I haven't had a chance to check up on her yet!

Would have been nice to have a second test of the BP before putting him on meds. Neko did some epic elevated BP's at the vet. First one was after a delay (vet emergency), barky dogs in the waiting room and a curious snout at her kennel (control your dogs people!), novice tech (or it seemed like that), and a BP over 220. We called it an epic fail, and I didn't get charged. Second attempt was at the acupuncture vet, a quieter clinic. Unfortunately a pup put his nose near her carrier while we were waiting. :rolleyes: Over 180 on the BP. Next attempt was at her dental. Dental vet put her in a room by herself for a while with Feliway's going - BP 141, nice and normal. Just as some cats have elevated BG at the vet, so too can vetty stress, or white coat syndrome, elevate the BP. After that if Neko was going to get her BP done, I sprayed her carrier with Feliway, gave her Rescue Remedy before we left and covered the carrier with a towel. 168 isn't horribly high and within margin of white coat syndrome BP.

Benazapril can elevate the Creatinine. Where it is beneficial, is if Darcy has proteinuria, and a urine/protein creatinine ratio test should be done a couple times first to confirm that. And the UPCR is usually done after seeing elevated protein in urine. Benazapril will lower the proteinuria, which is hard on kidneys so good to give if proteinuria is the issue. And side note, proteinuria is not uncommon in acros.
 
Yes. 4 cycles. Unfortunately it is a little hard to gauge since he’s at the vet a lot of the time. Tomorrow he will be home.
I have a home BP unit and I will try to get some readings on him. I agree about the Benazepril. I understand about the proteinuria which is what it’s used for in CKD cats (as opposed to HCM cats which it is also used for.). She never mentioned to me if she got urine from him. On Friday she said she was going to try. It should not be hard to get urine at some point from a cat that is receiving IV fluids. She also admitted that when they took his BP on Friday they had already taken blood (stressful.)
 
Urine from a cat on IV fluids is probably pretty dilute. Maybe they could try getting it before he's hooked up. Neko was on benazapril first for her kidneys, then later because of her heart.
 
Oh yes @Wendy&Neko I am with you about people and their dogs at the vet! How about the people at the vet with their dog sticking his snout in your cat’s carrier and the person says, “oh Rover just loves kitties; he has kitties of his own at home!” This has happened to me many times. I usually keep the carrier covered nowadays and in my lap! Although now with COVID we all have to sit in the car and wait.

Oh my goodness, what a crazy morning for you two. I can’t even imagine how you were emotionally handling this, I can just see you walking in the dark wondering what the heck was going on. :rolleyes: It sounds like he weathered the storm okay, hopefully you as well. I hope tonight he behaves, eats and keeps those numbers safe. Hope tomorrow is uneventful, Darcy continues to behave, eats, no vomiting allowed, poops and keeps those numbers safe. Sending loving, healing prayers for both of you, can’t imagine all that stress on your back. Prayers that jury duty people have mercy and send you home. :) Will be watching :D:D

YES! Nobody puts baby in the corner :D. I was very glad to see you had a nap. I have renewed appreciation of naps these days! Good luck getting out of jury duty on Monday.

I am giving Minnie's ears a bit of a break, just glad she's into yellow after yesterday's pinks. My happy place would be a nice blue spreadsheet - a bit like Mr Darcy's from yesterday! OH I picked up a can of tiki chicken with quail eggs yesterday and thought of you both.:bighug:
 
Just catching up. We lost power last night and didn't come back until I left for work, unshowered and hair not fixed. It was out over 8 hrs.

sorry to hear about Darcy. Ollie was on benazepril for hcm. Then a week before she crossed the amlodipine. And she got fluids. When the fluids were first started vet wanted to give 300 ml. I replied no way, she has hcm. We settled on 50. Then I got cardiologist involved and he wrote no more than 200 per week. I gave 70 three times a week. It was just a bit more, she did very well with it. Able to give her cerenia and b12 through it if she needed it. I used a burette chamber to give so it could be accurate.

Look at her dose, she is doing well in that regard.
 
What a long day for you, Suzanne. Your poor Darcy. I'm so glad that he is home today. It will give him a chance to recoup from all drama. I hope you are able to get out of jury duty. Sounds like Darcy needs you.

Yep. If you had given Olive that much fluid as the vet originally suggested, it is my opinion that it would have killed her rather quickly. Honestly, I am so afraid of giving fluids any more for any length of time, after what happened to my Tabitha. Of course, she had not had a cardiac ultrasound like Darcy just did. I would like to be conservative with the fluids, but the vets get very emphatic about it saying things like... his kidneys NEED that fluid .. it's urgent, etc.
