I haven't had a condo for a couple days. Tessie hasn't been feeling well, and I've just been waiting for her blood test results, which I should get later today.
I plan to discuss the use of prednisolone with the vet some more, as well as getting Tessie on bupe. It seems like something has changed with her, and in addition to being more listless this week, she is having stomach issues. LB is ok, but some burping after eating, and yesterday I noticed her lip licking for the first time. Her appetite stays great, though. Her BG is staying mostly in the 300's, although yesterday her nadir was 139, which was awesome to see!

Well, back to my painting, while I await the vet's call.

Definitely paws crossed on the test results. I'm so sorry, Sandy, to hear she is not feeling any better at all. Poor little girl!! She's really been through it lately. Will keep an eye on your condo for results.

hi guys .. sorry to hear tessie lou is not feeling 100% . I hope you get the test results in today so you know what is going on .. hopefully something a little med will take care of and get her good as new! Have a great weekend guys!

I would wonder about nausea also - classic sign is lip licking.

A lot of vets prescribe reglan/metoclopromide for nausea but it is not very effective for cats (very effective for dogs).

Ondansetron or dolasetron or cerenia (more for nausea with vomiting) would be the ones I would consider first. Reglan/metoclopromide can have side effects also.

Thanks everyone! The vet called late in the day, and with other stuff going on, I'm just now able to post again.

The good news is, the vet said her folate and cobalomine levels were normal--yay! The pancreatitis test however, was high. Her number was 6.3 I believe, and I think the top end of normal is 3.4? My vet got a free consult with an IM expert at Texas A&M, and she was able to discuss Tessie's situation. The IM guy said they still do recommend prednisolone even if they're diabetic--although he noted it's not the best situation and you just hope for the best outcome without side effects. He said not all cats blood sugars are elevated from it, but many are. The vet and I also agreed to start her on bupe 1.5ml bid, and Pepsid AC. Karrie, I'm hoping the Pepsid will help wtih any nausea she may have. It must not be too bad, cause she eats like a truck driver and has never thrown up! Thanks for the med names, though. I will keep them handy if things get worse in that department.

The first dose of bupe tonight went mostly on her head and my t-shirt. The second time, I squirted out half of it (not knowing how much, if any, she got the first time) and tried to give the other half to her. It ended up down the front of her and the back of my hand--which she very courteously licked off! Anyway, she definitely got some, because she was dozing in about 20 minutes, very soundly. That was about 3 hours ago, and she's still sleeping soundly. I will attempt a spot check at +6 (if I can stay awake).

I feel so bad for her, and she just stays her sweet, cooperative little self.

Well..at least now you know why she has been feeling so poorly. I hope tomorrow will be a better day for her. Get some rest, Sandy. Ni Ni.
 
Rocket weighs over 15 lbs and he gets bupe at only 0.20 ml every 8 hrs.....he has the 300mcg/ml compounding formula....it does make them sleepy at first...keep in mind it's like morphine hence the name buprenorphine....so will have the sleepy side effects....other than that i have not seen anything wrong with Rocket....

as per the pred....yes the neurologist did warn me about Rocket becoming diabetic (as he was on his way to a second remission...and he made me sign a waiver about it) but you gotta pick your battles and with Rocket's latest dx i opted to ignore the diabetes per se....you can always give insulin and up the dose.....but if Tessie Lou is having a flare up then i would suggest also learning how to give subq fluids.....to keep her hydrated...

lip licking and/or smacking is a sign of nausea and/or stomach acid....so pepcid ac regular strength (1/4 tablet a day to start) will be ok (it is bitter so do not give on it's own or dissolve in water to syringe...i would use a pill pocket or something to pill her if you can hide the pill) ...if that doesn't work then you can up the dose to bid....now if indeed is nausea and not acid then ondansetron is ok at 1 mg every 12 hrs....Rocket did not have a good reaction to the Cerenia

since Rocket has multiple issues he takes pepcid and ondansetron as well as the bupe and pred...among other meds....and yes his BG levels are up but to me it's worth working around the BG levels so long as he has good quality of life....

so i would say try the pepcid and yes...pancreatitis can be painful....so the bupe is good to give....

Re. the bupe -- it's supposed to go into the cheek pocket. Don't squirt it down Tessie's throat. I know you and Tessie both seem to prefer wearing the bupe but it's best absorbed via the oral mucosa. If it continues to be a headache, ask your vet if you can administer it via a sub-q shot -- just like insulin. It's likely the same liquid you already have. I'm not sure if an insulin needle is too thin to accommodate the bupe but you can ask or the vet can give you syringes with a larger gauge needle.

When Gabby has been prescribed Pepsid, it's been 1/4 of a 10 mg tablet (2.5mg) twice a day. The max dose is 5 mg twice a day.

You might note in the WSAVA meeting report, steroids are recommended only with autoimmune pancreatitis -- see the section on mild chronic pancreatitis. I haven't a clue how or if this can be diagnosed based on any clinical test or if it's based more on clinical observation.

Claudia is right. We can always manage the insulin around the steroid if Tessie's numbers rise as a result.
 
So glad you have some answers. The bupe I expect will make a world of difference. You are very lucky that she continued to eat well with pancreatitis. Give your Tessie Lou a hug from me.
