Yes, that's what I'm thinking too. He's getting pretty low. Even right now at +11 he's on the borderline. I'd be much more comfortable if his lows were a little higher.Solo, strictly speaking he hasn't earned a reduction, because as a lot nger term diabetic we look for a drop below 40, that said with the recent food change, cutting out the gravy food, bounces ate clearing quickly, and because we don't know if we are dealing with IAA or Acro, and with the sizeable depot from the 5u dose, I'm thinking it might be worth trying a reduction to 4.75, as long as ketones are still not a concern.
His health has been much improved. No vomit in days and he's been more active lately. No longer just sleeping all day and is having periods of walking around and getting into trouble. I also think I have figured out the feeding schedule. He's fine with the food now, I haven't had to do any coaxing and he eats it no problem, and 4 meals a day seems to be working. I noticed last night when he had 3 meals, one every 2h20min, the 2nd meal was untouched but the 1st and 3rd were gone. So I just took the same volume and split it between 2 today and it worked great. He ate the first meal within the first hour or so, was ready to eat again when second meal came, and was done with just about all of it by mid-cycle.What makes me a little nervous is the impact of the food change and the unknowns in his General health
So he's grazing,He ate the first meal within the first hour or so, was ready to eat again when second meal came, and was done with just about all of it by mid-cycle.
Sort of. I'd say he ate like 3/4 of the meal when it was served, comes back for a little more like 10 minutes later, and then finished up the rest a little later on. Meal times are +0 and +5.So he's grazing,
Getting a +1 and a +2 tonight might be a good idea since this is the lowest you have shot.Okay, dosed at 4.75 and he's eating now
Absolutely. I plan to get those at a minimum and then any additional I need based on what they are. If he bounces and starts to rise a lot I'll get some sleep, if he stays low I'll steer and retest as appropriate.Getting a +1 and a +2 tonight might be a good idea since this is the lowest you have shot.
I would give a little bit of lc food just to keep him up over 50 . See you at +2+1 54. Not much of a food spike, still pretty flat.
Good plan, glad to see that it was not lower. Did you feed a little food at the +1?+2 76. Still not as high as I'd like to leave him for the rest of the night, and I can't sleep for only 2-3 hours, so I'll just stick around and do a +4
Yeah, he ate a little more at +1Good plan, glad to see that it was not lower. Did you feed a little food at the +1?
I probably won't be on at +4. You know what to do to keep him safe. If he starts dropping again, you could always bring out some MC food so you can get some much needed sleep.
I'd give a couple more of gravy.54@+5.5. Should I give another tsp of gravy or just wait till +6 to see if he's coming up on his own?
Oh he's been eating. I didn't hold back his meal at all so he's had about 1/2 his normal meal over the past two hours, plus I couldn't get only gravy out of the little can so he had some of that food too with each serving. It still hasn't bumped him up much. I'm wondering if you've just been giving gravy with no food if he could do with something a little more substantial. Maybe half of his regular meal?
Okay I like that idea. Just based on his past history with DKA and ketones, I know I've got a little wiggle room there. He doesn't seem to build them up super fast and worst case scenario if things do jump up real high I've got the R on hand to bring him back down.I'm sort of thinking that with the 5u depot and the food change and if we have a situation of IAA breaking then it might make sense to drain the depot by shooting a one off depot reducing dose at amps if he hasn't bounced by then. 2.5 or 3u.
The decision to do that is made harder by the DKA, but equally with some of the variables you are dealing with if the insulin needs have dropped and we've got an overfull depot it could really see you struggle to keep his head above the water.
Dropping the dose like this is not without risk. But I'm thinking if he's still green at amps then the depot will keep him low for the first part of the cycle, and if he comes up high enough you could shoot up to an hour earlier at pmps to try to get you on a better shooting schedule.
That's true, but we would need to get someone to help you with that, I have no hands on experience with that, and I find it a bit scary.I've got the R on hand to bring him back down.
