? 3/5 Chino|AMPS=379|+4=437| PFS? NEED R ADVICE, PLEASE.

Yesterday...

Today is the 5th cycle on 7 units. We had 2 high, flat cycles yesterday. This morning, I gave 0.5 units Novolin R right after injecting the Lantus, while he was still eating. So I'm aiming for another 30 minutes of sleep before I have to test again.

I always run my fingers through his fur several times to make sure it's not wet after the shot, and it was NOT wet... but his BG is still going up, and his fur does smell like insulin. So I think I might've given a partial fur shot.

It has been 4 hours since his AM insulin. How soon can I give another dose of R, and how much should I give? I know that might be difficult to determine, not knowing how much Lantus is on board... but I REALLY don't want to see him go above 500 today. And since this is a human meter, he might already be above 500. Any advice would be greatly appreciated.

Is their any way chinos is getting into dry food?
I had this same problem with my cat and it turned out she was getting dry food from the other cats.
I'm using the same fancy feast chicken, the new food chart says the turkey is lower for carbs, I'll be trying it soon myself.
As far as giving more insulin, they will say no because it's 12 hours worth. On the possibility of a first shot, I too have down this. Now when I do the shots I hold her scruff tight and make sure I feel the needle go all the way in. I'm using the 30g x 5/16 syringes
 
I feed both cats canned food, and I watch them until they're both done eating - otherwise Chino will finish his food first and eat the other one's food.

In this case, I wasn't asking about giving more Lantus, but regular insulin, which has a faster onset and shorter duration. Chino has a high-dose condition and I've started using it in order to try to get him under 300 during the day, with guidance from others on the board who've also used it for cats with high-dose conditions.
 
I have no experience with R, so I can't answer your question directly, but just for more info-- I assume you'll be able to monitor all day, if you did give him an extra "boost" of R?

One tricky thing is that, as more time passes, you will (if you give another R shot) have three nadirs to keep track of-- Lantus, new shot of R and the "ghost" R furshot (just in case), so I'm guessing that if you don't get an answer on this soon it's probably a no-go :(. Hope Chino doesn't climb too high today.
 
+6 is about 1-2 hours from his usual nadir. If he's still above 400 when I check in ~30 mins, I think I'm going to give another 0.75 units of Novolin R, since 0.5 units didn't appear to have much of an impact this morning. I hate hate hate fur shots.

Thanks. I will be home to monitor. I'm pretty sure the 0.5 units of R made it in, and we should be past most of its action by +6. It's the Lantus that I smelled in his fur around +4. But even if most of it went in, his Lantus nadirs are almost always in the 300s. I tend to see a high PMPS followed by a steep drop when his AM cycle begins with a fur shot, so I hope that by bringing his BG back into the 300s for the 2nd part of the cycle, we'll see a smoother PM cycle.

Your plan appears to be well thought out. I like how you waited until +6 to give more R. This strategy can work well when numbers are high. I'm sure it's been mentioned... sometimes when a cycle is "disrupted/altered" by the use of R... the next cycle *may* run lower than you think. I'm glad you'll be around to monitor. Good luck!

Thank you for the feedback! I'm starting to get used to PJ parties :rolleyes: Since we have been seeing good results in the evening cycles, my plan is to only use R during the day.

Yes, we'll see. Stay on top of it since you really don't know what 0.75u R can do and you don't know what it will do given at this point in the cycle. There's no such thing as collecting too much R data. It's all part of your learning experience and eventually the data you collect will help you develop an R scale for Chino.

The thing that worried me somewhat was the fact that R can affect the 2nd cycle. Have yet to really see that with Bronx, but Chino does go lower at night, & by a lot sometimes.
 
Adrian, you have done an incredible job educating yourself, testing your kitty, and monitoring him closely. You are an amazing kitty papa and deserve a gold medal in kitty ownership! :cat::cat:

0.75 units R brought him down from 463 to 411 an hour later, but it was already creeping back up at +2. 3 more hours until the next dose of Lantus is due. I'll give his ears a break until then.

Tonight would've been the 6th cycle on 7 units but we're back at the beginning now. Oh, well. At least it will give me some time to learn how he responds to R while I count back up to 6 and wait to see if he'll need another increase.
