3/4 sid increasing dose - question on feeding

Sid's numbers have been too high and unpredictable the last few days so I am going to try increasing the dose of Prozinc. I have hc and test strips on hand. He is also showing increased level of ketones on the blood meter I have but still negative in his urine. I thought I might switch him to levemir but that ended up being a very bad idea because of his pneumonia. I just want to bring his numbers down more and I know I'm getting impatient so I will have to slow down. Once his pneumonia is cleared which can take weeks, I will reevaluate the insulin question.

I saw that you had switched to Levemir and did wonder about the lag time for the shed. Hope an increase on PZI will help you get where you want to be with Sid. And yes, patience is good. :-D

Having had a ketone scare last week, I'm the ketone test advocate! If your meter shows ketones in the blood, that is probably more accurate than the urine test, as it is still in his system and has not yet being eliminated. Keep a REALLY close eye on this. If it gets to anything more than TRACE or SMALL, it's an issue. And since his numbers are unpredictable, AND he has pneumonia, all of the ingredients are there. I just checked out those meters on line that test both yesterday - VERY cool! My Relion "asks" if I've checked ketones every time it registers above 300 - which is pretty much EVERY time! Unfortunately, it doesn't test for them.

The Vet School told me when they see a patient for DKA it's usually about $2-4,000. That wasn't in my budget, but testing, fluids and an increase in his insulin were. Hopefully he's still eating - I think that's one of the things that saved Grayson. That, and catching his "MODERATE" ketones right away. People that posted w/ me last Thursday night (2/23) on PZI were emphatic that we prevent him from getting to DKA. We were in to my regular vet first thing in the morning; have increased his insulin and have been shooting SubQ fluids since. AND testing for ketones at least twice a day! Think of it as an insurance policy!

Keep a REALLY REALLY close eye on those!

Patience is good, but it’s so damn hard! I feel for you Lydia.

Increasing is probably a good idea since you are seeing more ketones.

@Sue I was hoping he could build a shed at the same time the insulin brought his numbers down. He was on lantus before and the problem with that was the bouncing and unpredictable numbers. My hope was that the combination of medication to treat his diseases along with a different long-acting insulin like levemir might be the ticket to success. I was worried about whether or not he would even absorb the dose because I had a problem with that when he lost so much weight. I have been giving him his prozinc IM since mid January and he seems to respond to it though his numbers are higher than I'd like. Based on what happened today he either does not have the weight to absorb it or I gave way too low of a dose. But I don't really have the time to problem solve that right now with ketones on the horizon.


@Lu-Ann I totally agree with you Lu-Ann about staying on top of the ketones. I have been giving him fluids twice/day but I am thinking i will increase those a little too. He does not have a heart condition and he is on lasix for the pneumonia and that takes fluid out of the body so i think I will increase his fluid intake a little also. yes, and that cost for the hospitalization is the accurate range. Sid has been DKA so I know that very well. I'm going to do all I can to avoid going there again no matter what.

@Donna I'm not a very patient person when it comes to a sick cat. :smile: Any thoughts you can send my way to help with that is appreciated!!

I did increase his dose tonight to 1.6. I will keep a close eye on him. and test for ketones again later.

493 (4:45) I gave him about 1/3 can (maybe a little more) along with the .25 R
333 (5:45) I gave him a small bit - maybe tbs of turkey and giblets (authority)
282 (645) I gave 1/8 can and 3 treats - gave 1.6 U of Prozinc
301 (7:25)
263 (8:00) gave 2 treats
8:30 gave teaspoon of turkey and giblets
186 (8:45) gave 2 more treats

I'm trying not to let him get full but I'm trying to balance his dropping along with keeping the level stead. We have a long night ahead.
 
Thank you Sue. I'm trying to keep the meals on the small side so he doesn't get too full. I may add a tiny bit of karo syrup to his next feeding to slow him down a little.

Sorry about not posting the ketone reading. It was 2.9. But he still shoed negative on the urine strip. I tested that twice last night.

Thank s for the info it is helpful.
