Does anyone have an extra pair of patience pants I can wear? Mine have already worn thin! :-x

Yesterday's condo and numbers: http://www.felinediabetes.com/FDMB/viewtopic.php?f=9&t=114763

AMPS-352, .25 units
+5.5-140
+7-136
PMPS-407, .25 units
+6-404

Today:

AMPS-377, .25 units; switched from BD to Terumo syringe
+3.5-317

My DH and I bickered this morning. I was positive that there were a couple of air bubbles; he insists there weren't. I, of course, have no doubt that I am right! Calipers and a magnifying mirror should arrive tomorrow. Yea!

May you all have prettier numbers than we have had so far today!

Re: 3/4 Jersey AMPS-377, +3.5-317

Hi guys .. haha, you'll find that most patience pants are pretty thin around here, but thankfully someone always has a pair to borrow :-D I hope you guys have luck with the calipers .. have a great day!

Re: 3/4 Jersey AMPS-377, +3.5-317

I don't know if I've said hello yet, so if not, hi! I'm sure you were right on the bubbles  . Those silly things are so irritating! Hopefully the magnifier and calipers will make things easier for you. Hope your day is going good and Jersey is sliding down to some nicer colors :-D

Re: 3/4 Jersey AMPS-377, +3.5-317, +7-370; Humulin boost

I spoke to my vet today, and he told me to give Jersey a Humulin boost to help with the bounce. I gave her a breath (less than .10) of H at AMPS +7. Do you agree with the vet's advice? Did I mess up by doing this? I wanted to get your advice before I shot, but several factors prevented me from doing so. I'm watching closely and testing Jersey often at this point.

AMPS-377, .25 units Lantus
+3.5-317
+7-370, .10 units Humulin
+8.5-316
+9.75-280

Peter.... & Rachel ~ thanks for stopping by and giving me some words of encouragement!

Hi Shelly!
Just wanted to check in on you and Jersey :-D Hope the dosecrease to 0.25u does the trick for her. She does find those nice numbers but those darn bounces take over. I can't offer any advice about the humulin as I never intervened like that.
But yes do keep a good eye on her.
Have a good night!

Re: 3/4 Jersey AMPS-377, +3.5-317, +7-370; Humulin boost

Shelly --

Which kind of Humulin are you using -- N or R?

Regardless, it's OK to use this as a "bolus" insulin. However, there are a lot of "but's...." You need to do a curve on the Humulin. So, you've got to test Jersey every hour. N and R have different times with regard to their nadir. When using a bolus insulin, you need to make sure that the nadirs are not overlapping. Ideally, you do exactly what you did this evening -- give a tiny dose, do a curve, and see how Jersey responds. The Humulin should lower the numbers by 50 - 100 points so the Lantus can "grab on" more easily and do it's work to bring the numbers down.

I don't use a bolus insulin. I'm going to ask a few of the folks who have more expertise with this to stop by.

Re: 3/4 Jersey AMPS-377, +3.5-317, +7-370; Humulin boost

Thanks for checking on us, Kim. I sincerely appreciate it! The Humulin didn't seem to do much today - just barely got her into the yellow for a brief period. As I mentioned earlier in my condo, I'm struggling with patience right now ~ especially when I see how many blues we had on Humulin versus Lantus. I hope I made the right decision switching her to Lantus. nailbite_smile Lantus should come with a great big warning sign on the side of the bottle that states, "Take with patience" - kind of like medications say "Take with food." You all tried to warn me when we started......I just didn't listen very well!

Re: 3/4 Jersey AMPS-377, +3.5-317, +7-370; Humulin boost

Thanks for the information, Sienne. We're using N, which is originally what Jersey started out on. (She seemed to hit nadir somewhere between +3 and +5 on it.) It's now been 7.5 hours since the Humulin shot, and about 2.5 hours since her Lantus shot. As of 30 minutes ago, she was still hanging in the pink!

Any info anyone could provide about working two insulins at the same time would be greatly appreciated! I don't like it.....I don't like it at all. It "skeers" me! :-D

Re: 3/4 Jersey AMPS-377, +3.5-317, +7-370; Humulin boost

Shelly

I use Humulin R as a bolus for bounces. Sienne already pointed out many of the important things to consider. In addition, you don't want to shoot R as a bounce is breaking....only as the big rise of the bounce starts and when shooting it will not overlap nadirs. The big thing you do not want to cause is a steep dive and then a bounce from the R.

One other thing that many of us have noticed that use R is that you might see only a small response in the cycle you shoot it but it can be very effective at knocking back the counterregulatory hormones and causing the bounce to clear quicker so that the subsequent cycle or two, you might see lower numbers as the basal insulin (lantus or levemir) works.

The only member I know who uses Humulin N as a bolus is Dale. He is a very experienced member and knows how to use N. Here is Chip's SS. I wouldn't encourage you to use Humulin N because it is a longer lasting insulin than R and if you are just getting your feet on the ground with using lantus, using N is not the best idea.

Re: 3/4 Jersey AMPS-377, +7-370, +9.75-280, PMPS-306, +2-322

That's great information, Marje. Thank you so much for sharing it with me. I did use N (it's what we originally started with), so I'm glad you mentioned that it could be especially problematic. I'm also thankful that you mentioned I might not see the effects of Humulin during the current cycle - I was certainly looking for it this afternoon and didn't see much of anything!

I do have one more question - how do I know if a bounce is breaking? For instance, I can tell when Jersey started to bounce yesterday at PMPS. How do I tell when that bounce will end? Are there tell-tale signs, or is it just knowing your individual cat?

Thank you again. Words cannot express how grateful I am to all of you!

Re: 3/4 Jersey AMPS-377, +7-370, +9.75-280, PMPS-306, +2-322

Any time numbers start to come down like on the a.m. Cycle of 2/28 then jersey could potentially be clearing a bounce. Sometimes they start down and then go back up for a bit but they don't usually have a fast, high rise. So I would limit my shooting the bolus to just when she has been lower and zooms up. Shoot it on the fast rise as long as the nadirs don't overlap. Check every hour for 6-7 hours until you start to know her patterns with the bolus.

Also....never shoot the bolus insulin anywhere but the scruff because some cats have increased absorption in the chest, side, flank.

Re: 3/4 Jersey AMPS-377, +7-370, +9.75-280, PMPS-306, +2-322

Fantastic information - thank you, thank you, thank you. Hopefully, we won't have to do this again! One insulin is hard enough to deal with at a time!

Recap of the day:

AMPS - 377, .25 units Lantus
+3.5 - 317
+7 - 370, .10 Humulin-N (per vet, 1st time using bolus)
+8.5 - 316
+9.75 - 280

PMPS - 306, .25 units Lantus
+2 - 322
+ 3.5 - 273

Shelly

If you look at Gracies SS in my signature block and go to 2/27 a.m. cycle and you will see how to annotate the bolus. It's better to do it like that then in remarks.

Just as a recap, I would have been a little reticent to have you shoot N today. All of her numbers from AMPS to +7 were all within meter variance...not really up. Today, you didn't see a big change but as you use it more, you might find you have to start using it less or not at all. When she starts clearing bounces in one cycle on her own, you don't want to use R even on a fast rise.

If your vet had mentioned it yesterday, the perfect time would have been at +6 last night except no one wants to shoot a bolus at night  but...just as an example and as you did today (time of cycle was perfect). While the rise started at PMPS, shooting it at PMPS would have overlapped nadirs. You might find that using R instead of N might give you a little more flexibility on when you can shoot.

Good job!!!!
