I'm honestly not sure what to do with her dose. She's only on 1 drop BID. She dropped to 49 @ +4 today. Go down to 1 drop SID? She's eating better than she had been. She's gone back to thinking the pate is what she wants to eat and doesn't want the flakes (it's a day to day think for her). Should I skip her evening dose? See where she is and go from there? It's like she did fine for a few cycles and then went low again.

With that AMPS+4 being a 49, how do you reduce from here? A needle shot? I'm very curious how to get a smaller amount than a drop.
Maybe Kit cat's ready for an OTJ trial?!? Very interested in what the mods might have to say.

It's not just the mods who comment. As mods, our job is trying to keep things running smoothly on the board overall. Any person with experience on Lantus and TR is welcome to pop in and comment on what should happen next with Kit Cat.

Alicia, a few questions before I comment on dose. What is your feeding schedule for Kit? And what is her typical low carb food she gets daily? And lastly, how have you been measuring that 1 drop dose? The reason I'm asking is that she's seeing a lot more blues than we'd like to see in a cat ready for an OTJ trial. I am wondering if there is a way you can safely give her a bit more insulin support.

Thanks for the merge - I missed the one post per day rule.

Her feeding schedule this week and the next two weeks is different than it is most of the time. Usually, I make sure she eats food prior to her shot (she's never eaten much in a sitting, so I try to get her to eat as much as I can in that first one), then I go to work, and hope she eats during the morning. I try to come home around lunch (anywhere from 1030-1230) and test/feed more. Then I'm back to work until I come back home to pick up her food at 430 - although I try to be home to get her to eat something before I pick it up. Evenings I get her to eat some preshot (even if I end up not shooting), and then try to get her to eat before bed. Sometimes she eats over night, sometimes she doesn't. I also try to get her to eat some before I pick up her food in the morning.

Currently, the college I work at is on spring break, and is enforcing a second week of spring break next week, and moving to online teaching for a week the following week. So the next two weeks after this, I'll work from home in the morning, which makes it easier to try to get her to eat more. So she's been eating more this past week than she probably did before - but I was also overwatering her food up until about Sunday, and I was worried about her not getting enough calories, and filling up on too much water. Hence me forcing more food on her (I don't make her eat if she doesn't want it, unless she were low and needed the MC or HC food - and she hasn't turned that down the few times I've needed).

She usually eats fancy feast pate or fancy feast flaked. She works her way through a variety of flavors. She usually only eat's about a can and a half through a 24 hour period.

To get the 1 drop dose - I saw someone mention a way to push the plunger all the way in pretty hard, insert into insulin vial, then release the plunger (not pull back), and that's about a drop. So that is what I had been doing since I dosed down to 1 drop. There was another method that I hadn't tried with insulin, but had tested with water - test how many drops are in one unit (or half a unit), get it consistent, then twist the plunger until you get down to just the one drop.

I honestly don't think she's ready for OTJ either, I just don't know what the next step is.

Thanks! That's the time of day she's most likely to drop - it's when she dropped last week a couple of times. Mornings honestly work better for a drop, but don't tell her, or she'll switch it up just to mess with me. (HA!)

That should keep her a little higher and safer. And try seeing if she will consistently eat a tsp or two snack around +8 to +9. That might kick start the sputtering pancreas and bring down the preshot values. And maybe the next two weeks can be enough to get her ready to take off the insulin training wheels.

Thanks - I'll give that a shot. +8 or +9 gets a bit rougher if I'm in the shop during the afternoons (although the next four days not going into work at all will be a good time to experiment), but we can try for those after the pm shot and see the effect it has.
