2 meals per day question

Okay.. so I did glucose curve at home (using alphatrak) and sent to my vet and was expecting him to adjust insulin dose (we are giving 2U of Lantus twice now).
But then he called me yesterday and canceled our appointment and told me to feed my cat ONLY twice a day (breakfast and dinner, insulin time) -- no lunch between, then perform glucose curve for 2 weeks.

Is there any reason for it??? His level is usually between 470- 370ish. I was expecting maybe 2.5 or 3 units but our vet said he cant decide at the moment because we are feeding lunch.

The probelm is my cat does not eat a lot at once. He doesnt even eat full calories to be honest. What should I do?? We feed him at the exactly same time... even the lunch. Same amount, same time...
Since we are checking his glucose, weve been feeding him fancy feast pate for his lunch, no dry foods for lunch.

Not sure which is best. I dont want to lie to my vet either. But im not sure if I can call and ask him about 3 - 4 meals since he already kinda refused to adjust insulin dose for now.... im just super worried what if something happens before I see a vet again. He was diagnosed in June 28, so its been about 40 days. His level was 600 in the beginning, now highest is usually 470-450 then it drops to only 350ish after Lantus.
The lowest ive ever seen was 280.

Any tips??
 
Your vet is probably more familiar with older insulins I bet. With depot insulins two main meals and snacks work best. In fact those ready for an otj trial give a +9 snack to bring down the low blue bg. That only works on very low doses. Also one method I used as do many is to feed the curve to prevent drops under 50 if following the TR method to keep the cat surfing in normal numbers 50-100 longer.

We ask new people to post an introduction first on the main/health forum first and set up a signature and spreadsheet as we are very data oriented. Then you will be asked to choose a dosing method if you want dosing advice. We use two methods here but some do custom dosing led by their vet. If feeding a raw or canned diet you have a choice. If feeding any dry it’s SLGS you follow.

As you can see, we are strong advocates for feeding just low carb food, preferable just wet or raw, and home testing the blood sugars. Not only is home testing more economical, it is more accurate. Many cats are stressed at the vet and data from vet curves isn't very good. Since you are home testing (good for you!), you can use one of our dosing methods and learn how to change the dose on your own, with our help and without needing to contact the vet. Many of us don't deal much with the vet regarding the diabetes, other than getting a prescription from the vet, should one be needed where you live.

The good news is your vet started you on Lantus. The rest is not so good news. As you heard, feeding only twice a day is old style for older style insulins. Any food given mid day that is low carb will not substantially impact the blood sugar numbers.

Got it, thank you! Yes so, we went to same hospitals (just different location) but we switched our vet in the beginning. The first vet started us on Lantus, but we didn't really like the vet so we changed to other location and went to see another vet. So, I am not sure if he is familiar with Lantus or not, but yeah... Our vet rescheduled today's appointment since he wanted to see the curve with 2 meals/day. He didn't want to adjust doses yet either after he looked at the data that I sent him a few days ago.

We are using AlphaTrak3 and Accu-Check as well. If we don't get enough blood from our cat, we just use Alphatrak. He is about 420~450 in the morning (on empty stomach), after 2U of Lantus, it kind of drops but it doesn't drop dramatically. We tested his urinate and so far, he seems doing okay. I didn't want to increase doses without Vet's recommendation so we are planning to test his glucose levels for another 2 weeks by feeding him only twice a day -- not sure if my cat can put it up with that though.

And yes, we feed him with Fancy Feast Pate as lunch and his blood glucose doesn't even go up afterwards (stays in mid 300 or sometimes low 300), so I thought that would be fine. But since vet want to see our numbers after 2 weeks from now on, I guess we will follow his instruction for now?
