Hi everyone,

Jackson has been doing great on the one drop dose, and his last six cycles, he's been in the 70s at preshot.

My question for the "experts" -- I'm guessing that an OTJ trial is the next step, but my question is how to know when it's time in terms of having the most successful trial and being best for Jack's future health? I'm not in a huge rush if there would be benefit to waiting. I kind of had it in my mind that I'd like to see him at numbers well below 100 (like 75-ish) with 1 drop dose for a week or so, but that's just what seems good to me. :)

For the last few days, I've been giving him a snack at +9ish, and it seems to be helping, though last night, I gave it at +6 because I knew I wasn't going to be able to get up at +9 and it didn't seem to make much of a difference...still a nice AMPS.

Jack seems to be feeling great -- super affectionate (more so than pre-diabetes!) and happy and playful. I will post a pic of him one of these days so you can see what a handsome boy he is.

Actually, you have 6 and 1/2 cycles on 1 drop and perfectly normal non diabetic BG numbers.
Hold on, let's see what everyone else says... :)

I think you could start an OTJ trial anytime you want.
usually, we hold the 0.1u/1 drop dose for about a week (14 cycles). if everything looks good, start a trial at what would have been the next shot time.
if you want to get a few more days away from that last blue... that's fine. if you want to start tomorrow tonight... that's fine, too.

what would you like to do?

Thanks, all! I have a work meeting out of town on Monday that will have me away from home for a good portion of the day, so I'm thinking that maybe I'll start after that -- so last shot being on Monday night -- start on Tuesday morning? We'll see how things go between now and then.

Also -- quick question about the trial. I know that it's recommended that small meals are fed throughout the day. Is this just for the trial, or would I need to keep that up post-trial? And how frequently are we talking? Every 3-4 hours?

FWIW, his current schedule is:

AMPS: 3/4 can FF
+4ish: 1/4 can
+8-9ish: 1/4 can

PMPS: 1/2 can FF
+4ish: 3/4 can (for the last few nights, I've been giving only giving him 1/2 can at +4 and giving him 1/4 can at +9, but to some degree, I think that's what normally happens naturally -- he eats a good deal at +4 and leaves the rest for later in the night)

sarah, i don't think you'd have to feed him any differently than you are now during the OTJ trial.

for some reason i can't find my copy of libby's "official" otj trial instructions, but the gist is to test and then feed at would have been shot times. if the number is anything but green... retest in 3 - 4 hours to see if food brings him down. initially, small meals fed throughout the day are better than only feeding 2 large meals per day.

alex has been OTJ twice since she was diagnosed in 2006. for the sake of convenience i gradually went from several small meals per day to two - three meals. i say gradually because i was afraid to make abrupt changes. i didn't want to upset the apple cart.


i'll find someone who has those otj instructions handy. look for them tonight or tomorrow...

dyana just sent me libby's "official" instructions. thanks, dyana!

Test at your normal AMPS and PMPS times. Feed multiple small meals throughout the day as much as possible (small meals are less likely to overwhelm a newly functioning pancreas).

If Jackson is green at your normal test times, no need to test further until the next "PS" time, just feed small meals and go about your day. If he is blue, feed a small meal and test again after about 3 hours. If his number is lower 3-4 hours after a meal, then his pancreas is working!

If, after 2 weeks Jackson has stayed in normal numbers, we have a party!

Sometimes the trial doesn't work the first time and we have to give a little more support in the form of resuming insulin. It's not the end of the world if that happens, we just give him the support he needs. Our goal is a strong remission and it's better to take our time to get that than to rush into remission just to have it fail later on.
