We've been up all night because Ole went green!

Exciting times! I'm glad that today is a studio day and I don't have to interact intelligently with people! Ole is doing well! and so am I.

How do you decide when to do more than a ToR? What kind of data do you want to see to make that move?



I have been mulling about this question from Suzanne. I thought I'd give it a stab from my observing Ole, which is different from other R users. When I began using R, it was to bring him our of the penthouse floors. At the time, my main goal was to move his numbers down. I dosed him at .1uR, and he would indeed move down, hundreds of points. With several R curves, I could tell Ole was pretty reactive to a small amount of R. My intention then began to migrate more to using R not as a tool to lower numbers, but one to flatten out Ole's bounces. His pattern is very clear. Beginning at about +10, Ole begins to rise, usually quite dramatically. Combined with his food spike, I didn't feel that L was able to be effective until well into his dose. I don't know the reason for this rise, be it a typical bounce or duration issues, but to me it doesn't matter, he rises at +10. I decided to use R to combat that rise and flatten out his pattern. Knowing that .1uR moved him too much for my purposes, I began to use a very light touch of R, literally only coating the needle. I believe it has been effective in making L more effective.

This might be a little gabby, but I didn't get much sleep last night and it is early in the morning and I haven't brushed my teeth yet.

I thought I would add this to today's post from last night. It was answered in the wee hours.

I looked at Oles ss and saw he went from 431 to 73 in 4.5 hours on the overnight- :o

It was indeed a bold move.
Being that this is the lowest number you have shot R at, and considering the 358 point drop in the 4.5 hours between +3 to +7.5 last night, be attentive to monitoring today. You are in uncharted waters here.

Ole had a huge food spike this morning, but seems to be coming down from it. I am home for the rest of the day and can monitor closely.

Good afternoon!

All is well here, it nice to be at home without construction workers. This weekend we will be putting in the finishing touches in the bathroom.

Ole is fine. He came into the laundry rm when I was in there and left a nicely formed good sized healthy poo, how thoughtful and stinky. Loki, being a copy cat copied Ole, like, 2 minutes later. Double thoughtful stinkiness! Ole and I spent about 15 minutes outside sniffing around in the fresh snow wishing we were skiing. Okay, Ole was sniffing and I was wishing...

Ole seems to be cruising the pinks again with today looking a lot like yesterday. Any suggestions on dosing? Should I give R if Ole begins to climb at +10 or 11? Should I hold off on R to see how Ole reacts to the new vial? My understanding is that after R is 4 hours old in the body, it is gone and done, no depot. How then can numbers from days ago be referred to as "R induced"? Isn't R just a helper to L? Because Ole reacts pretty quickly to even a whiff of R, is that evidence of him not being insulin intolerant?

Uff-da, so many questions, still.

Thank you for all of your help and attention to my continuous questions. Curiosity cured the cat! or at least made him feel better.

I would skip the R if you are starting a new vial (based on you the history of the current vial you recounted)

True R is a fast acting insulin, with no depot and in most cases action is done in 4hrs.

We refer to numbers as "R induced" because without the R they would not have happened.

If there are IAAs present there are many variables that can come into play and must be considered-

• Injected insulin first gets bound to the antibodies. Any insulin that does not get bound goes toward metabolizing sugars. How much goes to antibodies and how much goes to metabolizing sugars is anybody’s guess and a moving target
• IAA can retard the initial rise of available insulin after an injection.
• IAA can lead to an increase in the half-life of free (unbound) insulin in circulation due to some bound insulin getting released back into circulation - The increase in half-life can lead to prolongation of action. Another moving target
• The release of insulin from the antibodies can happen at inopportune times

When using R to help L, sometimes it may take a few R free cycles to be able to judge what is going on with L and determine if L needs an increase.

Ole has seemed to cruise through the PS period in the pink (+10-+2). Perhaps soon he will cruise yellow!

We began a new expensive vial of Lantus this evening. We had about 1/4" or less in the old vial, but in a visual comparison, the old vial was definitely cloudy. My vet's instructions and FDMB's instructions did not match regarding care of Lantus, but I have talked to her about it, and she has seen the error of her ways.

Good luck with the new vial - I hope it brings sunnier numbers into Ole's SS. That sure was a spectacular night last night. :cool:
