2/5 Willie AMBG 304... thoughts?

Good afternoon, all! Happened to do our monthly hometest today, and found that Willie was at 304. :cry: :cry: :cry: We headed to the vet to get him tested, and unfortunately, we have to go back tomorrow to get the good urine test as his bladder was empty. Vet has given us a script for Lantus and wants us to start 0.5u BID (at my suggestion). She was originally going to have us start sub-q's too based on his BUN/CREA #s, but I pointed out that the high protein diet likely elevated those, and asked her to compare to his last labs. Turns out, his CREAT was better, and his BUN was about the same. Given his history of heart murmur, she decided to stay the course and not do the sub-q's for now.

One question for everyone... would you go ahead and start the Lantus prior to having the lab work back on his urine sample?

The vet didn't seem too concerned and wanted me to start right away. I pointed out that an infection could be elevating his BG#s and indicated I wanted the urine test pronto. She was all "oh, yeah. you do raise a good point." and then told me not to shoot if his BG was under 250. :roll: This was a new vet for us, and she was very nice and very supportive of our diet choices. She has a cat that is close to age to Willie (her "1997 model." too cute!). She was amazed at how healthy Willie was and how nimble he was on his feet.

Hoping our return from the Falls will be brief. Willie and Herbie have otherwise been doing great!
 
I think it's a tough call especially since you don't know how long his numbers have been up. Did she test for ketones? If the ketone test is clear (i.e., no ketones), you could wait for the blood work to see if there's an infection. A cystocentesis and C & S will take a couple of days for the culture. I would see where Willie's numbers are at tomorrow and if still high, I'd be thinking about shooting.

Did the vet check Willie's teeth? Dental issues can bring kitties back from the Falls, too.
 
Thanks, Sienne! Do they have to send out for the C&S? They have an in-house lab and did his blood work while we waited today. Wasn't sure if the same would be true for the urine test. No ketone test... I have the strips and am going to follow him around tonight. We'll be doing that with the urine test tomorrow as well.

Willie is a (mostly) toothless wonder, so dental isn't an issue. She did check his mouth and saw no cause for concern.
 
They may be able to do the C & S in-house. However, it still takes a couple of days for the bacteria, if any, to grow. The delay is getting the culture not sending the test out.

I thought I recalled Willie not having too many teeth left but I didn't want to assume!!
 
Hi! I hope Willie's return is brief! Ususlly they can test in house to see if bacteria are present, if there is they send it out for C&S to determine what the bacteria is and which AB is appropriate. If they find no bacteria present they don't send it out for further testing and it saves us a lot of money.
 
Hi Christie and Willie!!! Just stopping in to say I hope you aren't back for long - we miss you, but you know what I mean!! I hope all is well with Willie's tests, and if he goes back on the juice, it is ever-so-brief!!

How's Herbie?? We'd love to see pictures and updates!
 
aw, christie, i'm sorry willie is back! like Amy, it's always great to see you, but nobody wants you to HAVE to come back.

i probably wouldn't wait too long - just me. hopefully they'll get test results for you asap. was his white count elevated?
 
Thanks for stopping by, everyone! I'll be starting Willie tomorrow night, I think... can't make it to pick up the insulin to Costco until tomorrow at lunch. I tried finding another pharmacy closer to home that would break a box of pens, but no luck. The only thing off on his bloodwork was the EOS (not sure what that is). Everything else was in the normal range.
 
i'm pretty sure EOS is eosinophils, which measures allergies. maybe willie is being exposed to something that he's allergic to. punkin's got up to 14% in an april blood test when the pollen was out in full force. i was told that's nothing to worry about. my lab's normal reference range was 2%-12%.
 
Paws crossed there is an easily treatable issue causing the high BGs. I suppose everyone needs a little holiday from paradise every now and then. It is nice to see you and Willie can catch up with some old friends. We hope your stay here is a short one!
