Day 4 on this slightly fatter dose, and we’re finally starting to see some more consistent blues - yesterday was really good!

I do have some difficult news to share that isn’t diabetes-related, but Kai-related. We received the full histopathology results from the mass that was removed. Unfortunately, the earlier FNA diagnosis of trichoblastoma was incorrect. The mass was confirmed to be fibrosarcoma (injection-site associated sarcoma).

The pathology report did show clean surgical margins (9.3 mm peripheral, 2.4 mm deep), meaning no tumor cells were detected at the edges of the removed tissue, and no vascular invasion was present — all encouraging findings.

However, the tumor is considered high grade, with a high mitotic index (29) and areas of necrosis noted. Because of this, there is a measurable risk of local recurrence, which we will be monitoring closely. Based on the pathology and current clinical picture, the prognosis is guarded but hopeful, especially since the tumor was fully excised and there is currently no evidence of spread.

Right now, Kai is recovering well from surgery — his appetite is back, he’s acting like himself again, and we’re seeing his glucose start to settle back toward normal. For now, the plan is careful monitoring of the surgical site for any recurrence while focusing on his comfort and quality of life. Given his age, diabetes, and CKD, radiation is not something we feel is the right choice, and his vet agrees.

Tomorrow we go in for stitch removal, and I’m hoping we can start settling back into normal life again. I’m going to do my best not to let the worry of recurrence consume me, and instead focus on enjoying all the time I have with him — whether that’s a few months or five more years. For now, we just keep moving forward, one day at a time. ❤️ ❤️❤️

Safe surfing everyone🏄‍♀️❤️:cat:

WHAT!?! Oh God!! How scary. I'm so glad for clean margins--that sounds very positive.
Injection-site associated sarcoma...is this from the insulin or from another type of injection??
Oh, little buddy. Please continue doing well! I hope the stitch removal goes well tomorrow.
And I'm so glad to see him breaking from those reds and stinkies, down into some lemons and BLUES!!
Keep us posted if the vet gives you more details tomorrow.
Hang in there, Maddie!

Well, that's scary, but it sounds like you're handling it in a very positive and caring spirit. And I'm so glad Kai is continuing to feel better! ❤️

Oh Maddie I'm sorry to hear. How scary this all must be. I really do hope no reoccurance happens and Kai lives happily many many years.
I hope tomorrow's procedure goes well and glad to hear he's recovering nicely.
Boys and I send many many maaaaaaany healing vines your way

Sorry, I should have been clearer about what I meant by “injection site.” The short answer is no, there’s no known link between insulin injection sites in diabetic cats and fibrosarcoma. This type of sarcoma is associated with vaccination sites, most commonly with certain vaccines like rabies and feline leukemia. Even then, it’s quite rare. I think it its like a 1 out of 10,000 chance for it to happen.
