Day 4 on this slightly fatter dose, and we’re finally starting to see some more consistent blues - yesterday was really good!
I do have some difficult news to share that isn’t diabetes-related, but Kai-related. We received the full histopathology results from the mass that was removed. Unfortunately, the earlier FNA diagnosis of trichoblastoma was incorrect. The mass was confirmed to be fibrosarcoma (injection-site associated sarcoma).
The pathology report did show clean surgical margins (9.3 mm peripheral, 2.4 mm deep), meaning no tumor cells were detected at the edges of the removed tissue, and no vascular invasion was present — all encouraging findings.
However, the tumor is considered high grade, with a high mitotic index (29) and areas of necrosis noted. Because of this, there is a measurable risk of local recurrence, which we will be monitoring closely. Based on the pathology and current clinical picture, the prognosis is guarded but hopeful, especially since the tumor was fully excised and there is currently no evidence of spread.
Right now, Kai is recovering well from surgery — his appetite is back, he’s acting like himself again, and we’re seeing his glucose start to settle back toward normal. For now, the plan is careful monitoring of the surgical site for any recurrence while focusing on his comfort and quality of life. Given his age, diabetes, and CKD, radiation is not something we feel is the right choice, and his vet agrees.
Tomorrow we go in for stitch removal, and I’m hoping we can start settling back into normal life again. I’m going to do my best not to let the worry of recurrence consume me, and instead focus on enjoying all the time I have with him — whether that’s a few months or five more years. For now, we just keep moving forward, one day at a time.
