Recap of yesterday's BG readings

AMPS - 120, gave .05 units Lantus and 2 puffs 220mcg Flovent
+2 - 127
+4 - 116

PMPS - 153, gave .05 units Lantus and 2 puffs 220mcg Flovent
+2 - 203

WCR - Spirits are high. Did a little paperball and crinkle toy chasing yesterday and last night, no episodes of wheezing. Alert this morning. In the bathroom right now playing "chase the invisible bug" in the tub. Ate breakfast, left me a "gift" for all my hard work in the litterbox.

I am wondering since Cini's BG is inching up in to the yellow range if I should inch up his insulin too? Maybe go back to .10 units?

Hi Cathy

Remember that it takes Flovent a few weeks to reduce the inflammation. While you might not see any visible sx such as wheezing or coughing, the inflammation can still be there which could cause an increase in the glucose.

So a dose increase wouldn't be a bad thing to do in order to keep the numbers down a bit.

Sherry's Zoe has asthma as well. She adjusts the dose based on the kitties numbers. We can't be 100% sure the increase in numbers are due to inflammation with regards to the asthma or the pancreas pooping out....or some other reason. Drives a bean crazy, eh?

I would just adjust the same way you would normally.

Hope the dosecrease works! Tarragon is also on Flovent for asthma. He is also on a small dose of steroid pills every other day. It is a fine line you walk when trying to balance the two. Have a great day!

OK, I spoke to the Vet and she said it actually wasn't a bad idea to go ahead and start Cini on some Albuterol. She said to only use the inhaler when he had an active episode of wheezing.

Then she mentioned a medication called torbinutrol or toritrol, I didn't quite catch the name, that she wanted me to use on an "and/or" basis with the Albuterol. Anyone familiar with that and what I should "really" do with it? She was a little fuzzy on how I was supposed to give it and when.

The Vet also agreed that I should keep Cini on a maintenance schedule of Flovent, but she would rather see me taper him off after a month because she believes the long term steroid use benefits don't outweigh the risks of using it while having Feline Diabetes.

Albuterol is a rescue inhaler. It is a good idea to give a puff of albuterol before the Flovent to open up the airway while there is inflammation. I've never heard of the other. We do use albuterol when Tarragon is coughing to help the Flovent get in. How much Flovent is Cini on? Vets under prescribe Flovent a lot of the time. We use two puffs of 250 Flovent twice a day. The vet had only prescribed one puff of 100 Flovent once a day and it didn't touch it.

Since Flovent is an inhaled steroid, it doesn't affect BGs the same way as a shot or pill form. Flovent can be used long term. Hopefully Tena or Sherry will chime in as well.

Hi Cathy!

Possibly your vet was taking about Terbutaline? I am not real familiar with it, but I do know a lot of people who have used it with success in conjunction with flovent. Melissa mentioned, that albuterol is used as a rescue inhaler......I have also used it if Zoe is congested, she gets a puff of albuterol 10 minutes or so before her flovent to help open up her airways....this allows the flovent to get deeper into her lungs.

Flovent is really a safe drug for a diabetic kitty......the Flovent molecule is too large to be absorbed into the bloodstream, so there are no worries about it affecting blood sugar or having systemic effects like long term of steroids. Like Tena mentioned, it does take 2 weeks or so for the flovent to reach full effectiveness.

Zoe has been on Flovent for a little over 2 years.....she was OTJ when she first started on flovent and remained OTJ 16 months until a case of pancreatitis put her back on insulin 10 months ago.

It may be used up to four times daily or as needed for asthmatic cats already on daily steroids and every half hour, for 2-4 hours in a crisis. Terbutaline is in oral, injectable and syrup form and is almost identical to Albuterol, although many owners report the drug makes their cats spacey and/or disorientated. Terbutaline is also available in a child's syrup called Bricanyl® in the UK and Canada.

OK, so when would I use them both at the same time? Maybe I misunderstood her. Seems like I would just want to use one or the other, but not both.

Maybe she meant that if the Terbutaline made Cini jittery to stop and use the Albuterol instead. @-)

Albuterol can cause an increase in heart rate for an hour or so. I've used it a couple of times myself and noticed this. I think that's what the vet meant by jittery.

Here's an example of what I do if I see Curry coughing.

She coughs early in the morning....say 3:00AM....I will get up and give her one puff of the Albuterol....then at her regular Flovent treatment time....I might give her a puff of Albuterol....wait a a couple of minutes and then give her the Flovent. The Albuterol opens up the bronchiols and allows the Flovent to really get into the lining of the lungs. I might do the same thing for her evening Flovent treatment. Then I stop the Albuterol since I usually don't see/hear her coughing anymore. I continue giving the Flovent twice a day.

I agree with everyone. Keep CiniMini on the flovent - don't stop after a month. Any side effects are minimal and greatly outway the risks. There is no way I would have had Maverick on no Flovent.

The albuterol is used ten minutes before the flovent. It helps open the airways up so that flovent will work better. Albuterol is used on an as needed basis for when there are flareups/active symptoms.

Hi Karrie, thanks for the outline of how to use the Albuterol and Flovent. I just got the Albuterol inhaler this afternoon so I'm going to start using it tonight and will likely just use it one more time tomorrow morning.

I think I will start maintaining Cini on Flovent on a daily basis from now on. Now that I have that 4corners pharmacy resource, it will be much more affordable.

Out of curiosity I asked the pharmacy how much 220mcg of Flovent was going for now days and they quoted me a price of $250 :shock:

Now that I know about 4corners I can save that money for my kids college fund. :smile:

That is such a crazy price on the Flovent.

Interestingly enough, the Albuterol prices are pretty comparable bwtn New Zealand and the States.

The cost if Flovent in the US is crazy!! 4corners will definitely save you a ton of $. It will take about 3 - 4 weeks to get it though. Do you have enough on hand until then?

Hi Melissa, yes I have a new canister that I had just opened but only used 3 puffs the last time I used it. I'm placing and order at 4corners tonight.

Actually, that reminds me of a funny story - I could see by the caller ID on my phone that it was my boss calling me so I answered the phone in my cheeriest voice ever "It's a great day at Cathy's desk, how may I help you!" There was a 2 second pause and then my boss said "God, I almost threw up a little after hearing that." :lol: :lol: :lol: :lol:

Cini's PMPS was 140 and I felt a little hesitant giving him a full .10 units of Lantus so I gave him a fat0.05 instead. It's basically the plunger at the top of the last line on the syringe but not enough to show a sliver of daylight.
