This is an idea. It is only an idea. But, if it works, it might improve Samwise's glucose control.

Because of his variability, it might be helpful to layer on a shorter acting insulin (without the carryover of Lantus) to handle when he spikes erratically, so you could keep the Lantus dose constant (because it doesn't work well with inconsistent dosing), and use the shorter acting insulin for the spikes.

Humulin N will last about 6-8 hours in a cat, AND if you want, it can be compounded into a lower concentration if the vet will write an Rx For it. Humulin R lasts about 4 hours and also can be compounded into a lower concentration; it is harsher than N and brings the level down more abruptly.

What you might try, is when he spikes up above 400 (or 500, or whatever really high number you want to set), giving a miniscule amount of N - as small as you can reliably measure - and test to see how that works over the course of the cycle. The smallest amount I think I can reliably measure is about 0.25 units of the U-100 N or R.

Yes, this is layering 2 different acting insulins. You must be extremely cautious doing this and be able to stay with him during the cycle. You must test as you would for a curve - every 2 hours or if you observe something unusual. You must be prepared for a possible hypo.

I will note that some of the cats with acromegaly get doses of R to cover the upswing in glucose at the end of the 12 hour cycle. You may want to peruse some of their spreadsheets to get a feel for how they use it.
