Hello-

The string of nice numbers was short-lived.

The last dose of R Maxi had was 10 hours ago. Since then, he's been tested 4 times, and only the 4th time was his number high enough to warrant a dose of R. His PM, at which time he'll get 7.75 units of Lantus (except in the highly unlikely case that his number is way down), is only 2 hours away. Would it be right to give him R now, or to wait until his PM Lantus time? If I give R now, should I give 0.75 or 1.25? His current number of 302 warrants 1.5 of R per the scale given, but it's only a couple points from 0.75.

Since it had been over 10 hours, you could have given R. But that means no R until possible middle of the PM cycle. The amount of R you'd give would be the amount in the R scale for mid cycle for that number.

So I would have to wait until +6 of the next cycle, and not 4 hours after the R dose, which would be +2 of the next cycle?
 
One of the guidelines with R is to try to limit large drops due to the action of the two insulins happening at the same time, and to avoid the nadirs of the two insulins coinciding. We are still trying to get a better idea of Maxi's typical nadir, but it often seems he nadirs early, which would limit giving R to preshot or a bit before, or mid cycle.

Understood. So, 2 hours is more than "a bit before"? Should I limit R to PS time or within 1 hour before PS time? (besides at +6 of Lantus)

The other way to use R is when a bounce starts as long as you are absolutely sure it is a bounce and not just lost of duration. Because he’s only bounced once on 4/14 a.m. cycle, I’d keep this technique in your back pocket while you get a bit more data. You can also tag Wendy or me.
