131 glucose this morning and a clear puddle of puke

My boy has been running very high for the last 9 days. on 8/1 he hit 138 before shooting up to 513 when tested 18 hours after his last shot(Since I didn't give him one).
The vet had me lower his dose to .5u for about 3 doses and back up to 1u and his numbers stayed in the high 400's to the mid 300's for 6 days.
on 8/4 for his 6pm shot (513glucose) she had me up it 1.5u, at 9pm I took a reading and he was down to 371.
8/5 he tested 428 in the AM,438 in the PM,8/6 he was 429 in the AM and 260 in the PM. 8/7 he was 386 am/443pm
And Yesterday he started out at 262 in the am/392pm (Still on 1.5u)
Today he was at 131 and there was a pile of clear puke. I gave him his regular dose like the vet suggested.
I hope I did the right thing. The last time I skipped it and it threw him all out of wack for days.
Any advice or thoughts? (He is eating fancyfeast classics low carb)
 
Hi there,

Lantus is a shed based insulin. It has to build up before being released into the body. Changes in dose made this morning will not affect blood sugars for 2-3 days, not hours. When you skipped a shot, the shed emptied, and it took a couple of days to refill.

Usually dose changes are made in .25 increments every 3 to 5 days based on the lowest number in a 12 hours cycle.

Does this made sense to you?

Obie had a big drop in blood sugar at 131. Probably accounts for him not feeling very good.

I had a hell of a time dosing him at the beginning. I had these 33 gauge needles that were shorter and half the time the needle would bend. I switched to the 29 gauge and my shots have been more consistent (Although a bit more annoying for my kitty).
His second glucose test read 170 (Two hours after dose).
I think he is mainly the victim of a crappy home doctor.
I'm hoping his 6pm reading is around the same number.
Thank you for the response, I'm sure my boys body just needs a bit of consistency at the proper dose. :)
 
His 6pm reading was 263 and his reading this morning was 437. :(
He had a bit of Boiled chicken(boiled in plain water) last night for a snack.
I am wondering if the fact that at night he goes almost 9-10 hours without eating that his insulin is kicking his glucose too low and his body is raising it because of that?
We feed him at 6am while we test and shoot him up with insulin, 1-1:30 when I get home from work, 6pm when we test and shoot him, and usually a small snack before bed.
I try to keep his calories down by giving him modest portions. He eats mainly fancyfeast classics and boiled chicken and Tuna in water.
When I first started shooting him i was told to start at 1u, I did that and his numbers were still high but drifting down with 204 being the lowest and 480 being the highest
his average test were in the mid 300s to the low to mid 400s for 9 days.
During that time I got hold of different u100 needles that were shorter and thinner and caused more fur shots.
She had me raise his dose to 1.5u after that and his numbers rocketed from 386 to 508 over 3 days. during the last of those 3 days his blood hit 138 and I panicked and skipped his shot because I didn't want to hurt him, well his blood hit 513 at noon and the vet had me lower it to .5 u to see how he reacted, well his blood was between 446 and 476 for the next three test.
She had me go back to 1u and his numbers dropped to the 360-380s for the next three days. It was then that she suggested going back up to 1.5u.
His numbers level at around 428-438 for a day and started drifting down to the 260's with a few high numbers 393-443.
On 8/8 he was 262 in the am 392 in the pm. And 8/9 was his best day yet. I did three test on him. 131 am (Gave 1.5u shot) 170 two hours after shot,and 263 at 6pm.
His 6am number was 437. Now we did have a storm and he hates those. I am wondering if his 1.5 dose may be a bit too strong? Would a 1.25 dose suit him any better I wonder?
 
Hello! I can only tell you what I would do if it were my cat, but you have to decide whether to shoot or not, since you dont have a spreadsheet yet, I cant see the history. If a newly diagnosed cat test below 200 before shot time, amps/pmps..then it is advised to not give insulin, since you dont have enough data to know what he/she will do.If you do decide to shoot, then you might want to reduce, and then monitor/test closely, at least every hour. If the numbers drop below 50, be prepared to test every 20-30 min. while giving small amts of high carb food(or low carb, depending on how low he goes). If it were me, I would skip,not having enough info. Maybe someone else has better advice for you hopefully soon.

If you go to Tech Support, you will see a thread to link a SS (spreadsheet) to your signature. Follow the directions and if you have difficulty, you can ask for help.

Seeing Obie's numbers in the spreadsheet makes it easier to help you with dosing. My guess is that you might need to use some .25 doses. Sometimes a .5 increase is just too much.

Note: I don't post a Spreadsheet because I have had as many as 5 diabetic cats at one time. Currently, I am tracking sorianna on a Spreadsheet on my desktop. It is a very handy tool.
 
Great job on the spreadsheet! Now I understand what your post means. I was basing my reply on your subject line, which was from yesterday when you had the 131 and he had puked. You want to make sure they have eaten, and if there are issues with puking, then you may not want to shoot until he has kept the food down for a bit. They need to eat to slow down the absorption of the insulin. I think you just need to be consistent with the dose and not increase/decrease too soon. He has been on this dose for 13 cycles(2 cycles in 24 hrs.) He seems to have settled in to the 1.5, so I would give it a few more days if it were me. Also, if you cant be home during the day at his nadir(lowest point when insulin is at peak), then I would try to get a pmps+6/7 at a minimum. Most cats go lower at night without food on board, and it helps to catch those low #s. Dose changes are based on the nadir, not the am/pm #s. All of this info is available in the stickies threads. ECID(every cat is different) so take your time and gather as much data(test) as you can. I set my alarm at night to do spot check test and have caught quite a few low cycles. They can go low overnight, and then be sky high at amps, or go low mid day, and then bounce up at pmps. Hope this makes sense. Reading all the stickies really helped me to understand how Lantus works! And be sure to start a new condo(thread) every day with your amps # and a link to the previous days post if you can, as it helps keep the days/#s easier to follow. Obie looks good. i would not be concerned at this point whatsoever. He is doing what he should, at this point in the dance. Carry on.
 
Ths morning he was at 501. It seems yesterday he dropped about 38 points an hour until about 10 hours in and then shot up to 406.
He probably did the same thing last night. Ugh..

So glad to see you got the spreadsheet up and working. Congrats.

My guess is that if you are catching lows (under 70), you might want to lower the dose a quarter and see if the blood sugars will level off a bit.

Usually the lows will trigger the liver to dump glucose. That just raises the preshot numbers higher. Solution: test for lows at +5 to +7 and if low, lower the dose. But, ease it down, please. .25 is hard to dose but it is possible with 1/2 unit marked syringes.

Also, as a human diabetic, I did some research on Precose (acrabose). It is a human drug designed to block carbohydrates. Its major side affect is stomach upset. If Obie were my cat, I would not give this medicine. Cats are biologically obligate carnivores.
They don't metabolize carbohydrates the way humans do. So using a carb blocker doesn't make sense to me. Just my two cents after 38 years on insulin with 3 diabetic cats.
