To recap yesterday: lots of info to process from vet visit, he ate well last night, did not seem nauseous but I went ahead and applied Ondansetron transdermal to right ear @ +9; he's had 2 poops--small ones--since arriving home from the vet; I did not start the phosphate binder yet as my first priority is to see that he eats well, gets rehydrated, and stays below renal threshold but above hypo. Found it a little difficult last night to maintain a comfortable surfing range in the green. He ranged from 87 to 54 from pre-shot to +5 while I fed and tested frequently and he had his share of Karo water to bump him from time to time. He finally jumped to 122 and I felt he was safe enough for a few hours. Now we see if he bounces.
 
Sorry to bang on about this but please check with your vet about giving fluids daily. As also Ondansetron 2-3X a day and Famotidine every day. It's possible that the fluids make him feel better and you can reduce the frequency of the meds in time.
I won't need to check with the vet about it; if I decide to sub-q daily, I'll just do it. Wonder if 50 to 75mls daily would be a good choice. He just seems to leak out so much that I question whether 100mls is too much. As for the meds, I was considering alternating the oral Ondansetron with the transdermal--maybe oral @ AM+10 and transdermal @ PM+9 since I am up then anyway usually, testing and feeding. The Famotidine I could give once daily AM+5. I still haven't figured out the phosphate binder. Instructions are 1 scoop [which looks to be about 1/2t] every 12 hours mixed with the food. Sounds great to the researchers, I am sure, but not so good in practical, every day use, when the patient does not just eat once every 12 hours. Lately, Bama has been eating nearly every hour early in his cycles and maybe every three to four hours mid-cycle.

I don't think 100mls a day will be too much for an ~15 lb cat. We gave Bandit 100mls everyday for the first 3 weeks and she is 10lbs. Her crea came down from 3.8 to 3.1. We haven't tested after that. We're now giving her anywhere between 50-75 mls a day.

The phosphate binder came from the vet with instructions as to how much to add and when. I haven't checked Tanya's website on phosphate binders.
 
Thanks, Bhooma. I just got the Famotidine down him and syringed water several times so I think he swallowed it this time. Maybe I'll coat it in butter next time for a smoother down the throat ride. The first attempt he managed to move it just outside his mouth and it stuck to his lip. I've used a pill shooter but have not found that to be the solution. Maybe it just takes practice. I did use the shooter to feed water into his mouth as when I opened his mouth after the second attempt, I could see it still in the back of his throat. I'm certain that is what happened with yesterday's pill that ended up in his food bowl. I'll be testing and feeding in a few minutes so we'll see if it reappears.
 
Fed the rest of a FF can I'd opened sometime in the PM feeding hours, maybe @ PM+4.5. He's had a small amount of kibble in his pre-shot meals. Averages about 2 FF 3 oz. cans per day plus a little kibble. I am concerned about his getting enough calories. Yet I am feeding, it seems, all the time. Vet said he had lost about a pound since August which was a concern to her. Vet said there was a lot of protein in the urine and the urine concentration was "fairly impaired" at 1022. From my search on the web, I assume the number should be written as 1.022. I was writing down notes as we were speaking by phone and she said ten twenty two with no indication as to how that should be represented.

I thank all of you for your input and suggestions. This is a lot to process as I am sure you all know. Right now I am exhausted and just trying to determine what is the next thing to do. . . .like in the next five minutes or next 30 or next hour. I know I need exercise to work off all these stress hormones coursing through my body. I need sleep and to maintain good eating habits. I have to be in prime shape to make the best decisions for Bama and to cut down on mistakes and forgetting and getting all caught up in sadness. The vet did say it was not time to give up, that Bama still has some fight left in him.
 
Gina, I’m thinking about you and Bama. Hope you are able to spend some time resting and taking care of yourself today. I know how hard it is to feel it when you are mentally and physically exhausted — but in every word you write I see that you and Bama both have lots of fight left in you. Thanks, Jessica. Your words mean a lot especially knowing you and Callie have your own battles left to fight as do many of the people who post on this website. Or their battles are mostly over and either lost or won, and yet, they still come to post, to help, and uplift. And every kitty is worth fighting for. Their preciousness, their specialness is evident in their faces.
 
AM+8 100mls sub-q followed by kibble & FF snack. Bama has been sleeping in his old bed inside a cardboard box where it is safe and warm and dark. When I picked him up to move him to his sub-q receiving spot, he gave all kinds of vocalizations. The only time I usually pick him up from his bed is to put him in his carrier to go to the vet and he was having none of that today. He did settle down and receive his fluids with only a slight whimper when the needle went in. I inserted into the flank as suggested by @FrostD. Worked well. Did not even notice any leakage. :)
 
Looks like a bounce has started which may allow me to get some rest. Bama has stayed in his bed all day. He was eating well through mid-day; did not feed @ +9 since he was trending higher. Opened a new flavor of FF and gave a few bites which he seemed to like. Thought I would try mixing in the phosphate binder. Big mistake. He actually walked, got as far away from me as he could, up on his box next to a window and stared at me, flipping his tail. Since I had to give him his shot I went over and picked him up to bring him back. He protested when I picked him up. Got him a clean bowl, some FF & kibble covered in Squeeze-up and he started eating. Will test @ +2.
 
I wouldn’t worry too much about the leaking. I would do daily fluids. For Bama’s size 100 mL is ok. But if you want to experiment with 75 mL daily then do it. I’m just afraid that if you only give 75 mL and he still leaks out a lot, then he won’t get very much. But why don’t you try it and see if it makes a difference. You won’t know until you try. The every other day fluids won’t stay with him long enough to rehydrate him. It’s very important to rehydrate him. Very important. And he won’t be able to poop if he doesn’t get rehydrated. He is at risk of becoming very blocked up if he stays dehydrated - and then damaging his colon from it. He also can’t feel better until he is more hydrated because of the uremic toxin buildup. Sorry. I keep typing stuff and then backspacing over it because I don’t want to be too alarming. But I have a lot of personal experience with this in my cats and I get upset because I don’t want Bama to have bad things happen.
 
Aluminum hydroxide is supposed to be tasteless and odorless, but they know it’s in there. At one point I was putting it into capsules. When I switched to the renal foods I didn’t have to worry about binders much anymore. But you can bump up the protein for them by using egg white powder which has protein but not phosphorus.
