Still not budging; dose increase to 5.5 U this morning. (No R because of the increase.) It just occurred to me that if his dose is going up the R scale might need to get upped again as well. Something to think about, at least.

Nope, he's just not budging. Got another 1.25 U of R this evening, and at +4 he's doing 12 points from PMPS. Ketones are up now, too... 2.5 at pm +4. So I'm glad I gave him the R, even though it didn't seem to do much yet. I think it's time to bump the R up again. The past few doses of 1.25 U have barely budged him.
