Oh, sweetie....I feel so bad I can't even express it. I want to say something wise and comforting but I just don't know what. Do you and your vet have a course of treatment planned?

Don't know much yet.
Vet thinks the oncologist (a former colleague) is gung-ho on Palladia. Never heard of it, so am Googling. Seems like it's fairly new and has been used on dogs, but not so much on cats. Allegedly good for aggressive tumors like Butthead's.

There are numerous factors to consider in cancer treatment (regardless of species), such as:
purpose (comfort/palliation, stabilization, remission/cure)
cost (monetary AND emotional/mental stress)
time (ex. travel, treatment durations, recovery durations)
discomfort (nausea/pain)
possible complications
extent of disease - metastasis is about a stage 4
current health status of the patient
how well the patient copes with medical procedures.

I posted elsewhere about Pepper, one of my household who developed osteosarcoma. Note that the type of cancer is important; my experiences with Pepper may not apply to a different kind of cancer.

when Cosmo was sort of diagnosed with lymphoma or sarcoma, Dr B told me that cats respond REALLY well to chemo/radiation. He assured me that they don't get nearly the side effects us humans do, plus their course of treatment is much shorter. I think that the oncologist tomorrow will let you know the best options, but you have to let them know what your financial limitations are... i used to ask Dr B what he would do if Cosmo were his cat and money were no object. If we were only considering Cosmo's happiness and his quality of life.

Did they say anything about margins? i mean... metastatic... does that mean that they know there are other tumors somewhere else? I guess these questions will get asked or answered tomorrow.

It kinda went like this: they took Butthead to that Secret Back Area to remove his stitches, and when the vet came back with him, she mentioned his appt with the oncologist tomorrow. I said I wasn't sure what to expect since I don't know his biopsy results yet. "Oh, it metastasized to both nodes. I thought I called you." Um, no, you didn't :-|. Thanks for dropping a bomb on me like that. While the news wasn't completely unexpected, I was too stunned to think and ask coherent questions, so I don't know how much of the margins were removed. The oncologist was sent the report and should be able to answer that.

AAhhh...MJ...not the news we all wanted to hear and you are being so brave about it. Even if you have your suspicions, the actual words (esp when delivered in the way they were) sort of knock the breath out of you. We're so sorry and we hope the oncologist can shed some light on positive treatments that will help him. Sending many healing vines and tons of snowflakes....since we have them today!

Very brief update.... at tbp now and the sooner I finish what I have to do here, the sooner I can get back home to Butthead. We decided to try Palladia. It's oral chemo given every other day. Might have to experiment coating with butter, cream cheese (a favorite snack), or pill pockets. I don't think pilling him will be simple. Cannot crush and give in food.

MJ, you should be able to request and receive a copy of the pathology report from your vet, if you're interested in reading it. I hope the oncologist answered your questions.

We were unable to pill Meowzi a couple of years ago when she had to take a chemo pill (cytoxan). We made arrangements with our vet techs and showed up at the hospital and they'd take her in the back and pop it down her throat, then they'd bring her back out and we chased with a small snack. Maybe you can ask your vet if the techs can do it for you.

MJ, I'm just seeing the news about the cancer. I'm so sorry to hear this. I'm glad for you and Butthead that there's a treatment that sounds promising. Good luck! 12/30 Donovan's buddy Butthead @oncologist

Headed out to Angell Animal Medical Center in Boston this morning with Butthead. I plopped him in the litterbox just before we left, and he dutifully piddled. Such a good boy. He is accustomed to shorter trips, so 10-15 min in he was like Are We There Yet? Let Me Outttt. Butthead has a barely perceptible meow, so it was mostly him headbutting the sides of his soft carrier. He was next to me on the passenger seat, so I gave him lots of scritches.

Didn't get lost finding the place. Let's just say I've had several very long adventures in the Boston area before (no GPS), so was happy not to have to backtrack through those often nonsensical street patterns. Had to wait a bit after checking in, and then we were escorted to an exam room where a technician got his weight, temp, and heart rate, and then asked for a brief summary of the events that brought me and Butthead there. I believe my theory about Butthead and authority figures who have a Y chromosome is correct. He wasn't thrilled with this guy, yet all of the techs (females!) at the VCA where he had his surgeries just loooove him, and he loves them back. He hates his regular vet too, who also has the Y chromosome issue. Bottom line is, apparently Butthead really likes women.

I let Butthead wander around the exam room while we were waiting for the vet. He walked on the counter/exam area, sat on the office chair, sniffed the printer, turned around and did happy pee-pee tail to everything, and.... decided to investigate the sink. Maybe he was thirsty? Unfortunately, the faucet was one of those motion-activated things, and he set it off :shock:. He didn't get too wet, but he did avoid that area afterward. Finally the vet and another technician arrived (female!).

***Note to anyone who has a cat with potato farms in their ears like Butthead: apparently ears that are actively producing "stuff" and/or become inflamed or irritated on a regular basis are more prone to cell mutations. I never thought about it that way. Just thought Dang, some cats always have dirty ears. Try to keep an eye on them.

Given that Butthead's ear canal ablation showed clean margins, and whatever was left behind grew back so quickly, the vet recommended either carboplatin (IV chemo) or Palladia (oral chemo). From what I could understand, carboplatin is used more for existing tumors, and Palladia is better for keeping bad cells from getting to that point. While carboplatin would be effective more quickly vs. Palladia which takes several weeks to build up, carboplatin requires more visits for treatment and more monitoring. I would not have any issue with that if it was the only deciding factor. But there is very little data on use of Palladia in cats, except that vet says it seems effective and well-tolerated. Vet asked if I wanted some time to think about it, but I felt the sooner we start, the better. Palladia could not have been started any sooner because of its mode of action -- it would have affected his healing after surgery. Could also theoretically use BOTH drugs, and/or switch to the other if it appears not to be working. Vet tech was leaning toward Palladia. I decided to try that first. Once Butthead starts the drug, he will have to have his blood tested in a week (CBC, which I can get done locally), and vet wants to see him again in a month. Also have to watch for side effects, mostly stomach upset. I mentioned dietary changes (like maybe finally get rid of ALL the HC food and kibble in the house? - theory that sugar feeds cancer cells), but she did not seem concerned aside from giving him good quality food. I'm still going to lobby for removal of the Kitty Crack though. That crap isn't good for any of them.

Butthead and his women.... vet had her laptop on the counter and Butthead lay down beside it and stretched out his paw on the keyboard :roll:. They left and she typed up a summary of the visit to give me. I'm cheating and looking at it now -- very good idea because who ever remembers everything the doctor says?

Drive home took much longer than the drive out. Sucky traffic for no visible reason; typical Massachusetts. Butthead wanted OUT of his carrier, so I relented and set him free, but I put his harness and leash on just in case. He's really good though. He likes to look out the windows, hang out on the back seat, and most of all my lap. He was purring, silent meowing, and making happy feet. This is not an ill cat! I was completely covered in Butthead fur by the time we got home.

I have yet to give him a pill. Have to see how that goes tonight. He does NOT like anyone futzing with his mouth, so I think I will have to combine it with something pleasant like nummy fuds. I wonder if the others will catch on like they did when it was Donny's pokey time. They discovered he got a treat then, and darnit, they wanted one too.

It sounds like a really really nice vet visit at Angell. I'm glad Butthead liked his vettybean!!

As for pilling.... did you ask vet if there was an injectable form of Palladia? I always asked. Most vets don't think that any owners would be more comfortable injecting vs pilling... but they don't know about the vampire club!

If you must pill... the pill pockets work ok. Some tricks: make sure to follow directions on the bag where you don't use the same fingers that touch the pill to smoosh up the pill pocket - avoid transferring smell of the pill. Also, you can try smashing up some kibble or dry treats and roll the outside of the pill pocket in them... kind of like coating chicken for frying.

Cosmo got wise to this though.. .he started eating around the pill (he must've chomped on the pill once). So then I started giving him some treats, then the pill pocket, then more treats. He was a greedy treat eater and snorfed them down as quick as possible, unless he was having a p-itis attack.

to answer a few questions here before creating a new Butthead thread:

Palladia is only available in pills, and since it has to be given every other day, bringing him to the vet's office to do that is kinda unreasonable. Last night he had his first pill, and I coated it in cream cheese. After I jammed it in his mouth he was like Waltz Tango Foxtrot? but then got distracted when I offered him more unadultered cream cheese. Hopefully he won't figure out the little game. Donovan hid from me when it was pokey/shot time at first, but he got over that.

No obvious side effects yet, but don't know how soon they would show up.

Your thoughts and prayers and encouragement mean more to me than you could possibly know.

get a long, skinny pill popper. cleo was a monster to pill, and you have to remember chemo shouldn't be touched by others so it's just easier to place pill way at back of throat so he doesn't spit it out. follow immediately with a squirt of water so it doesn't irritate his throat.

we used every other day chemo also (leukeran at one point, then cytoxan).

just be careful to watch him, especially to see if he looks like his legs hurt. palladia basically works on cancers that are carried along by blood vessels (or are highly vascular) so cats on it can get blood vessel abnormalities. also watch for GI trouble (bad diarrhea), for which you may need metronidazole (flagyl). many cats do much better on palladia than dogs do, but the doses often have to be adjusted. so far i've heard it works best on mast cell tumors and neuroendocrine tumors, but it's been tried on pets with all kinds of cancers. butthead's not on prednisone or prednisolone, is he? if so he needs to be watched even more carefully for gastointestinal trouble. lots of oncologists are very keen on palladia right now, and i'm hoping it does a great job for butthead.

Thank you for the input. Yeah, I handled the pill with gloves, and informed anyone who removes gifts from the litterbox not to do it bare-handed ( :shock: - no worries, we use a scoop and clumping litter). I'm going to get a pill popper in case I end up needing it. Butthead is not on any other meds.

pilling fail.

Well... crap. I just found the pill I gave Butthead yesterday spit out onto the scratch box near where he was sleeping. I gave him more cream cheese after pilling, and then he washed himself, so I don't know how or when the heck he expelled it :? . It was quite intact aside from the outer coating being partially dissolved.

I gave him another one. He wasn't happy about it. Gotta find a better and more successful way of doing this. If Petco is open tomorrow, I'll see if I can get a piller thingy.

Uh oh. Butthead is one smart kitty. Sorry about the pill ejection button he pushed. I guess he enjoyed his cream cheese and spit out the 'pit' as if it was a martini olive. :roll:
