12/1 farine's OTJ failure

Can you please look at Farine's SS in the print screen above and tell me if you guys would increase the dosage right away?
Also could there be something wrong with Farine? I did try an OTJ trial as you guys suggested in a previous post, but not only the trial fail, but Farine has reached BG levels that she hadn't reach in a while.

Alain sent me a PM asking me to take a look at Farine's SS. He also asked if there may be something medical going on.

Can you please update Farine's regular spreadsheet with the numbers you posted? It's hard for me to read the numbers in the attachment.

With regard to something being wrong, the first thing I always think about is whether a dental is needed. Many cats lose good regulation or start having BG problems if they have gingivitis or some other dental-related problem. If the vet is able to get a very good look at Farine's teeth -- and this is hard in a cat that's not sedated -- and there's nothing wrong, then we need to look for other explanations. If Farine has not had an annual check-up with blood work, etc., then it's always good to get that taken care of.

It may also be that Farine needs a tiny bit of insulin to help support her pancreas.

Sorry about the delay! I'm in a foreign country with a bad connection and an iPhone and Farine's US spreadsheet's code is broken so I have to convert all the numbers and then write them down... Not an easy task!

Sienne, Farine had a complete dental procedure one year ago. They did extract two teeth and removed tartar from her teeth.

Couple of weeks ago, they found that Farine had a urinary infection and a little redness on her gum. Alain gave antibiotics to Farine and had another cystocentesis and blood test ran, so now we know that her infection is gone.
Now as for her redness, the vet said that she did not want to practice an anesthesia on Farine for 2 reason :
1. She is 15 years old
2. She has a stage 2 CKD

She said that the redness didn't look bad enough to risk an anesthesia on Farine.
I personally didn't know what to encourage Alain to do cuz I knew it could be risky to let her keep that tooth/gum problem with her diabetes and CKD, but who am I to encourage him risk a dental procedure on his cat...

If we were 100% sure that the tooth problem the vet saw is the cause of her diabetes getting unstable, I'm sure it would encourage him make that decision, but what if its just a periodical thing caused by the OTJ trial that we shouldn't have tried ...

I don't know what to say...
And now about the next injection... I dont know if he should stick with .20u and wait or increase de dosage...
 
i think i would stick with the 0.2 dose for a few cycles and get some tests in to see what it's doing, then decide if it needs to increase. with just 2 shots after a few skipped ones, we probably need another 5-8 shots to see what this dose is able to do.

sorry about coming back on insulin. i imagine that's discouraging, but you'll get her back under control again.

not sure what to say about the dental. i can understand the concern. often infections with the gums or decay are the cause of a cat needing more insulin or coming out of remission and it doesn't usually go resolve if the teeth aren't taken care of. do you have veterinary dental specialists available?

i took punkin to a specialist because of other health concerns when he had his last dental done. there was an option of having an anesthesiology specialist present if i'd wanted it. he also had early CKD and some heart concerns. He did fine with the dental. I guess i would explore to see if that is an option, and if that would make it safe enough for Farine to have a dental.
