Nice blue mid-cycle in the p.m. cycle and a pretty healthy drop but not enough that should account for the high preshots. If he was bouncing he should have settled by now as long as you've held the dose.
Got a question for you. Instead of trying to catch random nadir checks, do you have a day of the week where you can run a full curve and test every 2 hours or at least a mini curve where you test every 3 hours? Like a day off or a weekend day/night? Full curve would be the best but a mini would fill in a lot of missing data. If you can, what I would do with all you have on your plate right now is get preshots up until curve day, maybe a few just random spot checks, then run a curve and see if perhaps his nadir isn't where we think it is and he's either dropping really fast early or dropping farther than we think, or just plain flat as a pancake with occasional dips.
Although I suspect his dose just isn't high enough yet, and because he's staying so high he's building up a little glucose toxicity that we need to break, once we hit that breakthrough dose he'll respond better to lesser doses fairly quickly. Sometimes you have to go up in dose to come down. It's like there is a wall that you have to breakthrough before you can find the ideal dose.
