This is how Cini's day went yesterday:

Pepcid (NS) @ 7am
Fed 1/2 can FF Classic Salmon Feast @ 7:30am
AMBG - 198@ 8am
Metoclopramid (NS) @ 8:45am
Fed other 1/2 can FF Classic Salmon Feast @ 9:30am
+4.5 BG - 155@ 12:30pm
Fed 1/2 can FF Classic Beef Feast @12:45pm
+8 BG - 202@ 4:30pm
Fed other half FF Classic Beef Feast @ 5pm
Gave Pepcid (NS) @ 6:30pm
+11 BG - 177@ 7pm
PMBG - 178@ 8pm
Fed FF Classic Chicken Feast @ 8:10pm
Metoclopramide @ 9pm


I am thinking maybe the high +8 reading is because the +4.5 reading is the lowest he's been since OTJ ?

Happy Day 3! I must admit that Cini Mini has me scratching my head! I keep waiting for movement one way or the other....most kitties that look like this when they start an OTJ trial do one or the other...go up or go down. These are not normal numbers, but he is holding himself basically flat.

So far, so good....carry on! :-D

Isn't it odd indeed?

Today I even kept him on a feeding schedule of not feeding till after I took the BG reading. So, this morning's AMBG was somewhat a fasting glucose because the bowl was empty when I got up this morning at 7am. Normally I would feed him and then do the AMBG an hour later at 8am, but today I wanted to get him on more of a regimented schedule because I plan to have a pet sitter come by twice a day while I am on vacation during Christmas and New Years.

Today, so far, has been:

8am - AMBG
8:15 am fed breakfast (waitied 30 mins to allow Cini to eat)
No food in the mean time
11am - +3 BG
11:15 am lunch (waitied 30 mins to allow Cini to eat)
No food in the mean time
3:00pm - +7 BG (this is +4 from the last feeding)
3:15pm snack (waitied 30 mins to allow Cini to eat)
No food in the mean time

I plan to do another BG in 2 hours so that I have a +2, +3, and +4 after each feeding.

So, I now have three after eating readings:

+2 - 221
+3 - 181
+4 - 185

Next reading will be Cini's PMBG at 8pm tonight so that will be a +5 after eating reading.

Cini's PMBG (224) wasn't much better than his +9 reading.

Am I right in thinking that if it's anywhere in the 200 range tomorrow morning I should go ahead and start him back on the insulin?

And if so, do I pick up where I left off with .10 units?

if Cini were mine, I would be wanting to give some insulin. He's actually surprising me with how well he's doing and he might be able to keep going, but it looks like he could use a little help.

I would probably start with 0.1u and see what it does. You have asked about controlling his drops with carbs, and I think that is definitely worth experimenting with. There are several people here who use mini meals to help control their cats' cycles and it can really help in combatting the early drops.

I don't necessarily feel comfortable answering your question, but Muffin failed her first OTJ trial, and we were using ATrak also. Once she started hitting yellow, I did cancel the trial and resumed with .1u. If you want to take a peak at her ss, it was 8/31/10 we started the trial and resumed insulin on the pm cycle of 9/4/10. I might wait for the wise ones to chime in, tho, before I made a decision.

Thanks Libby. I think tomorrow Cini and I will have a long talk and go back to the Lantus bottle :?

sounds like a plan, unless he surprises you with a great AM number.

This is why we call it an OTJ "Trial." Sometimes the trial doesn't work the first time. We just keep giving them whatever they need.

FYI, here are a couple of other spreadsheets that have insulin responses similar to Cini's. Both of these cats went OTJ, but it took some work and a good bit of experimentation with shots and with carbs. Interestingly, both of them are now back on the juice again. I don't know if that has anything to do with their body's response to insulin, or maybe because the pancreas produces inconsistent amounts of insulin, or if it is just coincidence. They did both get off insulin and stay off for a while, though.

just wanted to cheer you on a bit. i imagine it's discouraging, but he's so close i think he'll get there, even if it's not just yet. hang in there!
