More of the same with these numbers, yuck. I just did a urine ketone test and it looks a bit darker than yesterday:

Might just be because it was pretty damp. It still looks closer to the 0 color than the other colors.

I did a control here myself, which was a lighter "tan" in color:

Just curious to see if I should be concerned with this. I think I have the knack of urine sample down again, but if it would be better just to get a blood meter to be safe, I can do that as well, just might take a few days to get here.

Today was the third cycle after the increase, so planning on holding for 6 total before jumping up again but just want to make sure I stay on top of the ketones, since she's been above the renal threshold for 9 days now.

Hi Seth,

I don't have any experience with the urine strips but for me it looks like something between the 0 and 0.5 mmol/L, which would be still very good. I know many people here use the urine test strips but I feel better with the blood tests. Ketones show in the blood first before you can trace them in the urine. And this guessing which colour it is would make me crazy. Good idea to test on yourself! As Bell had some ketones at diagnosis, I would think about getting a blood meter. But maybe that is just my OCD-mind.... :facepalm:

I use the strips too, Seth. It “appears” like it’s fine from the photo.
Of course what Heike said, using the blood meter is more accurate. She’s completely right.

Hoping Bell decides to come back down again. Safe surfing pretty girl :bighug::bighug:

You are right, I just need to get my butt in gear and order one.

I do have a question for you, Heike: Binie spends quite a bit of time above the renal threshold. Do you see ketones creep in with her? Or does insulin help prevent that from happening even though she stays above the renal threshold? The reason I ask is that Bell was averaging around 250s before even starting insulin and was in pretty rough shape with her ketones and weight loss. The few weeks she has been well above 250. Just trying to get a gauge on how quickly I could see her ketones start creeping up. If you’re not sure, no problem at all, was just curious on Binie’s experience in that regard.

As far as I know, it varies if cats are prone to have high ketones with high blood sugar. Fortunately, Binie was always in the normal range (up to 0.6 mmol/L) even though her bg was high most of the time. I identified 0.1 mmol/L to 0.3 mmol/L as her "normal" range while being unregulated. The only time things got really out of control was after her surgery, when she had a huge inflammation in her body. Ketones went up quickly to 1 mmol/L. I know that is not high for some cats but for Binie it showed that something is seriously wrong. Also when she had her UTI problems, the ketones went up. But that is Binie. For other cats it can be different. Some cats have higher ketones as long as they are not regulated and they are o.k. with it.

So it might be, the higher bg didn't do much to Bells ketones. Her situation is different today than while she was diagnosed: She gets insulin, even though that doesn't give her the numbers you would like to. And insulin, especially lantus, reduces ketones. So from my non-expert perspective, it is good to watch the ketones but I don't see a reason to panic.

I understood that high ketones come from three things: not enough insulin, inflammation and not enough calories. You give insulin, seems her anal gland is under control (thank god!) and I never read that eating is a problem.

Maybe you want to tag one of our experts for adivce too. This was just my 5 cents... :)
 
Heinke is correct, it varies by cat whether there are ketones at high blood sugar. Neko was a lot higher (reds/blacks) than Bell, and for a lot longer, but still tested negative with the test strips each time. I've also seen cats in the yellows test positive for ketones. You need to learn your cat.
